目的:探讨能量多普勒诊断烟雾病颅内狭窄和闭塞血管的临床价值。方法:利用CDE技术对17例烟雾病患者的102条颅内主要动脉(大脑前动脉ACA、大脑中动脉MCA、大脑后动脉PCA)进行观测,并与DSA结果进行对比。结果:CDE在诊断烟雾病颅内血管狭窄和闭塞方面具有较高的临床价值,其灵敏度(Se)分别为86.27%、66.67%,特异度(Sp)90.20%、92.22%,准确度(Ac)88.24%、89.22%,假阳性率(α)9.80%、7.78%,假阴性率(β)13.73%、33.33%,阳性预告值(+PV)89.80%、53.33%,阴性预告值(-PV)86.79%、95.40%,阳性似然比(+LR)8.80、8.57,阴性似然比(-LR)0.15、0.36,约登指数(YI)0.76、0.59。结论:能量多普勒在诊断烟雾病的血管闭塞和狭窄方面有较高的临床价值。
Objective: To investigate the clinical value of energy Doppler in the diagnosis of intracranial stenosis and occlusion of moyamoya disease. Methods: A total of 102 major intracranial arteries (ACA, MCA and PCA) of 17 patients with moyamoya disease were studied by CDE technique and compared with DSA results. Results: CDE had high clinical value in the diagnosis of intracranial vascular stenosis and occlusion of moyamoya disease.
