目的:研究实验性颈动静脉瘘(AVF)的血流动力学特征。方法:14只成年兔均形成颈AVF并于瘘形成三个月时将瘘结扎,于术前、瘘形成一月、三月及瘘结扎后一月在不同位置行经颅多普勒(TCD)检测,以观察血流动力学变化。结果:实验动物AVF于瘘形成后三个月仍通畅者11只,在瘘动、静脉处TCD显示高流速、低搏动、紊乱的血流频谱,伴随响亮的撞击样血管杂音,以瘘口处血流变化为著。瘘结扎后,血流速度明显下降搏动指数恢复正常,血管杂音消失。结论:AVF形成后TCD具有典型的频谱特征,其变化程度与距瘘口的位置相关。TCD用于AVF的诊断及观察血流动力学变化具有很高价值。“,”To investigate the hemodynamic features of experimental carotid arteriovenous fistulae(AVF)in the animal models by transcranial Doppler(TCD).Methods:The AVFs of experimental models were surgically constructed in 14 adult rabbits.We measured and analyzed the velocities,pulsatility index(PI)and spectrum waveforms at different position and duration of fistula formation.The TCD findings were divided into before 1 and 3 months after fistula formation and 1 month after AVF closure.Results:Eleven of 14 fistulae still existed 3 months after surgery.The blood flow with higher velocity and lower pulsatility,marked turbulence and bruits in the artery and the vein of the fistulae were detected.The closer to the AVF the more typical features of the abnormal flow.There were significantly different in the velocities and PI between pre-and post-AVF formation(P<0.001).Conclusions:TCD is useful for evaluating and detecting the typical hemodynamics in AVF happened in clinical practice.
