目的检测 Toll 样受体-2(Toll-like receptor-2,TLR-2)和 Toll 样受体4(Toll-likereceptor-4,TLR-4)mRNA 及蛋白在慢性鼻-鼻窦炎、鼻息肉和正常下鼻甲鼻黏膜组织中的表达及表达程度的差异,探讨其在慢性鼻-鼻窦炎和鼻息肉发病机制中的作用。方法采用反转录聚合酶链式反应(reverse transcriptase-polymerse chain reaction,RT-PCR)分别检测10例慢性鼻-鼻窦炎筛窦黏膜、10例鼻息肉和10例下鼻甲黏膜组织中 TLR-2和 TLR-4 mRNA 的表达;免疫组织化学技术检测20例慢性鼻-鼻窦炎筛窦黏膜、20例鼻息肉和20例下鼻甲黏膜组织中 TLR-2和 TLR-4蛋白的表达和分布,并比较表达程度的差异。结果①TLR-2、TLR-4 mRNA 及蛋白在慢性鼻-鼻窦炎、鼻息肉和下鼻甲组织中均有表达,免疫组化显示蛋白主要表达在鼻黏膜上皮细胞及腺体细胞朝向鼻腔或腺体管腔一侧的细胞膜;②慢性鼻-鼻窦炎组织中 TLR-2 mRNA 及蛋白和 TLR-4 mRNA 表达水平高于鼻息肉和下鼻甲组织,差异有统计学意义(P 值均<0.05)。③鼻息肉组织中 TLR-2、TLR4 mRNA 及蛋白表达水平与下鼻甲组织相比差异无统计学意义(P 值均>0.05)。结论 TLR-2和 TLR-4在慢性鼻-鼻窦炎、鼻息肉和正常下鼻甲黏膜上皮细胞膜上均有表达;慢性鼻-鼻窦炎和鼻息肉组织中 TLR-2、TLR-4 mRNA及蛋白的表达量不同,提示 TLR-2和 TLR-4在两者的发病机制中起不同的作用。

Conclusion TLR-2 and TLR-4 are expressed in the membrane of chronic rhinosinusitis, nasal polyps and normal inferior turbinate mucosa epithelial cells.
