目的探讨内皮素-1(endothelin-1,ET-1)对支架植入术后无症状患者冠状动脉病变进展的预测价值。方法:测定252例行造影随访的无症状患者的血浆大ET-1浓度,研究对象于6-9个月前因慢性稳定性心绞痛行经皮冠状动脉介入和药物洗脱支架植入术。结果:进展组(n=93)和非进展组(n=159)间基线临床特征无差异。进展组血浆大ET-1浓度显著高于非进展组(0.86±0.26vs0.52±0.15fmol/mL,P=0.001)。多元回归分析显示,血浆ET-1可独立预测原发性冠状动脉病变造影显示的快速进展(OR=2.094,95%CI1.047-4.608,P=0.037)。结论:血浆ET-1是支架植入术后无症状患者原发性冠状动脉病变造影显示快速进展的独立预测因子。
Objective To investigate the predictive value of endothelin-1 (ET-1) on the progression of coronary lesions in asymptomatic patients after stent implantation. Methods: The plasma ET-1 levels were measured in 252 asymptomatic patients who underwent angiographic follow-up. The subjects underwent coronary angioplasty and drug-eluting stent implantation in patients with chronic stable angina 6-9 months earlier. RESULTS: There was no difference in baseline clinical characteristics between the progression group (n = 93) and the non-progression group (n = 159). Plasma ET-1 concentrations in the progression group were significantly higher than those in the non-progression group (0.86 ± 0.26 vs. 0.52 ± 0.15 fmol / mL, P = 0.001). Multivariate regression analysis showed that plasma ET-1 independently predicted the rapid progression of primary coronary angiography (OR = 2.094, 95% CI 1.047-4.608, P = 0.037). Conclusions: Plasma ET-1 is an independent predictor of the rapid progression of primary coronary lesions in asymptomatic patients following stent implantation.
