目的 通过对I型糖尿病并发酮症 (DK)、酮症酸中毒 (DKA)的住院患儿进行血清 β 羟丁酸及其他生化指标的检测 ,确立血清 β 羟丁酸测定在该并发症临床诊断及疗效观察中的重要性及意义。 方法 对 4 4例I型糖尿病并发DK、DKA住院患儿采用全自动酶分析法测定血清 β 羟丁酸等项目。 结果 I型糖尿病并发DK及DKA患儿发病初血清β 羟丁酸 10 0 %增高。其中DK组测定值 0 .4 1~ 5 .6 9mmol/L ;中位数为 1.92mmol/L。DKA组测定值 0 .5 2~ 6 .5mmol/L ,中位数为 4 .12mmol/L ,两组间差异有显著意义。病人在接受胰岛素及其他相应治疗后血清 β 羟丁酸在 3~ 8d内迅速降到正常水平。 结论 血清 β 羟丁酸检测在I型糖尿病并发DK及DKA的临床诊断及疗效观察中显示出其特有的灵敏度和特异性 ,应成为该并发症进行实验室检查时与其它生化项目及血气分析同样不可缺少的指标
Objective To detect the serum beta hydroxybutyric acid and other biochemical indexes in type I diabetic inpatients with ketosis (DK) and ketoacidosis (DKA), and establish the serum beta hydroxybutyrate assay in clinical diagnosis of this complication And the importance and significance of the observation of efficacy. Methods Serum β-hydroxybutyric acid (HSA) was determined by automatic enzyme assay in 44 pediatric type I diabetic patients with DK and DKA hospitalization. Results Type I diabetes complicated with DK and DKA early onset of serum β hydroxybutyrate 100% increased. Among them, DK group had a value of 0.41-1.56 9mmol / L and a median of 1.92mmol / L. DKA group measured 0.52 ~ 6 .5mmol / L, the median was 4.12mmol / L, the difference between the two groups was significant. Patients receiving insulin and other appropriate treatment of serum β-hydroxybutyrate in 3 ~ 8d rapidly dropped to normal levels. Conclusion Serum β-hydroxybutyric acid test has shown its unique sensitivity and specificity in the clinical diagnosis and curative effect observation of Type I diabetes mellitus (DK) and DKA. It should be the same as other biochemical projects and blood gas analysis in the laboratory examination Indispensable indicator
