目的观察比索洛尔对老年永久性心房颤动并慢性心功能不全患者心肌能量代谢的影响。方法 48例老年永久性心房颤动并慢性心功能不全患者随机分成2组,分别给予比索洛尔(实验组)和地高辛(对照组)治疗12个月,治疗前后采用多普勒超声心动图检测并计算心肌能量消耗(MEE)及常规结构指标,左心室收缩(LEVF、LVFS)舒张(IVRT、EDT)及整体功能指标(Tei指标)。结果 2组治疗前后,左心房内径(LA)无明显变化,与治疗前相比无统计学意义(P>0.05);2组患者治疗后心功能均有改善,有统计学意义(P<0.01);实验组与对照组相比,在RWT、LVMI、LVM、Tei指数、cESS、MEE/sys、MEE/min、NT-proBNP等指标变化明显,具有统计学意义(P<0.01)。结论比索洛尔可改善老年永久性房颤患者左心室重构和心功能,可以明显降低心肌的能量消耗。
Objective To observe the effects of bisoprolol on myocardial energy metabolism in elderly patients with permanent atrial fibrillation and chronic heart failure. Methods Forty-eight patients with permanent atrial fibrillation and chronic cardiac insufficiency were randomly divided into two groups, treated with bisoprolol (experimental group) and digoxin (control group) for 12 months respectively. Before and after treatment, Doppler echocardiography Myocardial energy expenditure (MEE), routine structural parameters, left ventricular systolic (LVVS) diastolic (IVRT, EDT) and global function (Tei index) were measured and calculated. Results Before and after treatment, the left atrial appendage (LA) had no significant change, compared with those before treatment (P> 0.05). The cardiac function in both groups improved after treatment (P <0.01) ). Compared with the control group, the indexes of RWT, LVMI, LVM, Tei index, cESS, MEE / sys, MEE / min and NT-proBNP in the experimental group were significantly changed and statistically significant (P <0.01). Conclusion Bisoprolol can improve left ventricular remodeling and cardiac function in elderly patients with permanent atrial fibrillation, and can significantly reduce myocardial energy expenditure.
