目的评价采用动态增强MRI(DCE-MRI)定量监测血管阻断剂治疗兔VX2种植性肝癌的效果。方法成功建立28只兔VX2种植性肝癌模型,以数字法随机等分为2组,分别给予血管阻断剂M410(治疗组)和生理盐水(对照组)。给药前(基线时间点baseline)、给药后4h、1天、4天、7天、14天分别行MR平扫及DCE-MRI,并于每个时间点取肿瘤组织行病理检查(HE及CD34免疫组化染色),观察两组肿瘤体积、对比剂容积转移常量(Ktrans)、各时间点肿瘤Ktrans变化率(ΔKtrans)及病理结果的变化。结果治疗组肿瘤较对照组生长缓慢,给药后第4天、7天、14天,两组肿瘤体积差异有统计学意义(P<0.05)。治疗组Ktrans值于给药4h后降低,至4天达最低,随后逐渐恢复,于14天时接近处理前水平。给药后4h、1天、4天治疗组的Ktrans值较对照组明显降低(P<0.05),余各时间点Ktrans值两组差异均无统计学意义(P>0.05)。两组各时间点ΔKtrans%变化趋势与Ktrans值变化趋势相同。DCE-MRI血管功能参数值改变与肿瘤组织HE及CD34染色结果相符。结论 DCE-MRI可动态定量评价血管阻断剂治疗兔VX2种植性肝癌后肿瘤组织的变化,是肿瘤血管靶向治疗后监测和评价疗效的理想的影像学手段。
Objective To evaluate the effect of dynamic contrast-enhanced MRI (DCE-MRI) on the quantitative monitoring of vascular occlusive disease in rabbit VX2 implanted liver cancer. Methods A total of 28 rabbit models of VX2 implanted hepatocellular carcinoma were successfully established. They were equally divided into two groups according to the digital method. The vascular blockers M410 (treatment group) and saline (control group) were given respectively. MR imaging and DCE-MRI were performed before administration (baseline time), 4h, 1d, 4d, 7d and 14d after administration, and the tumor tissues were taken for pathological examination at each time point And CD34 immunohistochemical staining) were used to observe the changes of tumor volume, Ktrans, Ktrans changes and pathological results at each time point. Results The tumor growth of the treated group was slower than that of the control group. There was significant difference in tumor volume between the two groups on day 4, day 7 and day 14 after administration (P <0.05). Treatment group Ktrans value decreased after 4h administration, reached the lowest level of 4 days, then gradually recovered, at 14 days close to pre-treatment levels. At 4h, 1d and 4d after treatment, the Ktrans values in the treatment group were significantly lower than those in the control group (P <0.05). There was no significant difference in the Ktrans values between the two groups at any time point (P> 0.05). The change trends of ΔKtrans% and Ktrans of two groups at the same time point are the same. DCE-MRI parameters of vascular function changes with the tumor tissue HE and CD34 staining results. Conclusion DCE-MRI can be used to evaluate the dynamic and quantitative evaluation of vascular blocker on the changes of tumor tissue after VX2 implantation in rabbits. It is an ideal imaging method for monitoring and evaluating the curative effect of tumor vascular targeting therapy.
