三叉神经心脏反射（trigemino-cardiac reflex, TCR）是指在涉及第五对脑神经三叉神经的手术操作中，当心率和血压突然低于基线值20%的临床现象。听神经瘤切除术中常会因为三叉神经附近的手术操作和其他机械刺激而导致TCR，表现为突然心率减慢、血压下降甚至心脏停搏。此例患者为一名52岁女性，在全身麻醉下行右侧听神经瘤切除术。肿瘤切除后，关颅前使用硫酸新霉素溶液冲洗时患者突然出现心动过缓，继而出现心脏停搏，术者停止操作后患者恢复心脏搏动，整个过程为15 s左右。通过此病例讨论术中TCR的相关因素，加深对TCR的进一步认识与思考。“,”The trigemino-cardiac reflex (TCR) is a clinical phenomenon where heart rate and blood pressure suddenly fall down below 20% of the baseline during surgical procedures involving the fifth pair of trigeminal nerves. During acoustic neuroma resection, TCR often appears due to surgical operations and other mechanical stimuli around the trigeminal nerve, which is characterized in sudden bradycardia, hypotension and even cardiac arrest. In the current report, a 52-year-old woman underwent right acoustic neuroma resection under general anesthesia. During the surgery, bradycardia suddenly occurred when neomycin sulfate solution was used to rinse the brain before cranial closure, followed by cardiac arrest. As the surgery stopped, heart beat was recovered. The whole process was about 15 s, the patient gradually recovered after the operation was stopped. Based on the case, we discussed related factors of TCR during operation, so as to deepen the understanding of TCR.
