目的探讨彩色多普勒超声对乳腺纤维腺瘤和分叶状肿瘤的鉴别诊断价值。方法回顾性分析经病理证实的乳腺单发纤维腺瘤患者233例(A组)和乳腺分叶状肿瘤患者20例(B组)的彩色多普勒超声表现。结果 A组肿瘤最大径、肿瘤边缘呈分叶状比例低于B组,而肿瘤内部回声均匀比例及BI-RADS分级为3、4A级的比例高于B组(P<0.01)。A组肿瘤内部血流多呈0级或1级的比例高于B组(P<0.01)。两组肿瘤内钙化发生情况、纵横比和阻力指数比较差异均无统计学意义(P>0.05)。结论超声图像显示直径较大、分叶状特征、内部回声不均匀、囊性变以及血流分级为2、3级的肿块是乳腺分叶状肿瘤与乳腺纤维腺瘤鉴别诊断的重要指标。
Objective To investigate the differential diagnosis of breast fibroadenoma and lobulated tumor by color Doppler sonography. Methods The color Doppler echocardiographic findings of 233 patients with breast fibroadenoma confirmed by pathology (group A) and 20 patients with lobular tumor of breast (group B) were analyzed retrospectively. Results The maximum diameter of tumor in group A was lower than that of group B in the edge of tumor. The average ratio of internal echo and BI-RADS in group A was higher than those in group B (P <0.01). The ratio of blood flow in group A with grade 0 or grade 1 was higher than that in group B (P <0.01). There was no significant difference in aspect of in-tumor calcification, aspect ratio and resistance index between the two groups (P> 0.05). Conclusion Ultrasound images show that the large diameter, lobulated features, uneven internal echo, cystic degeneration, and blood flow grade 2 and 3 are important indicators for differential diagnosis of breast lobular tumor and fibroadenoma of the breast.
