目的研究白细胞分化抗原90(CD90)和趋化因子受体4(CXCR4)在肝细胞癌组织中的表达及其与术后早期复发的关系。方法采用免疫组化法检测140例肝癌组织标本中CD90和CXCR4蛋白的表达情况,并分析二者之间的关系及其与肝癌术后早期复发间的关系。结果肝癌组织标本中CD90阳性表达率和CXCR4高表达率分别为80.71%和61.43%;CD90和CXCR4在肝癌中的表达均与肿瘤的大小、结节数、血管侵犯和TNM分期相关(P<0.05),而与年龄、性别、甲胎蛋白(AFP)和肝硬化均无关;CD90与HBsA g状态和肿瘤分化程度相关(P<0.05),而CXCR4与肿瘤包膜相关。CD90阳性表达和CXCR4高表达呈正相关性(r=0.394,P<0.001)。单因素分析结果表明,肝癌早期复发与肿瘤大小、结节数、血管侵犯、TNM分期、CD90和CXCR4蛋白的表达程度均相关(P<0.05),而与患者的年龄、性别、HBsA g状态、AFP、肝硬化、肿瘤分化程度和包膜无关。多因素分析表明,血管侵犯、CD90蛋白阳性表达和CXCR4高表达是肝癌早期复发的独立危险因素。受试者工作特征(ROC)曲线结果提示,CD90和CXCR4能预测肝细胞癌患者的早期复发,ROC曲线下面积(AUC)分别为0.648和0.657。结论 CD90阳性表达和CXCR4高表达与肝细胞癌术后早期复发有关,并可以作为肝细胞癌早期复发的独立预后因素。
Objective To study the expression of CD90 and CXCR4 in hepatocellular carcinoma (HCC) and its relationship with early postoperative recurrence. Methods Immunohistochemical method was used to detect the expression of CD90 and CXCR4 in 140 cases of hepatocellular carcinoma. The relationship between the expression of CD90 and CXCR4 and the relationship between the expression of CD90 and CXCR4 and the early recurrence of liver cancer were analyzed. Results The positive rates of CD90 and CXCR4 in HCC tissues were 80.71% and 61.43%, respectively. The expressions of CD90 and CXCR4 in HCC were correlated with tumor size, nodules, vascular invasion and TNM stage (P <0.05 ), But not with age, sex, AFP and cirrhosis. CD90 was correlated with HBsAg status and tumor differentiation (P <0.05), while CXCR4 was associated with tumor envelope. The positive expression of CD90 was positively correlated with the high expression of CXCR4 (r = 0.394, P <0.001). The results of univariate analysis showed that the early recurrence of HCC was correlated with the size of tumor, the number of nodules, vascular invasion, TNM stage, CD90 and CXCR4 protein expression (P <0.05), but not with age, sex, HBsA g status, AFP, cirrhosis, degree of tumor differentiation and capsule. Multivariate analysis showed that vascular invasion, positive expression of CD90 protein and high CXCR4 expression were independent risk factors for early recurrence of HCC. The receiver operating characteristic (ROC) curve results suggest that CD90 and CXCR4 can predict the early recurrence of hepatocellular carcinoma, the area under the ROC curve (AUC) were 0.648 and 0.657 respectively. Conclusion The positive expression of CD90 and the high expression of CXCR4 may be related to the early recurrence of hepatocellular carcinoma and may be an independent prognostic factor for the early recurrence of hepatocellular carcinoma.
