[目的]探讨染色体核型分析在血液系统恶性疾病诊断、治疗及预后中的价值。[方法]对358例血液系统疾病患者的骨髓细胞经24h短期培养后,按常规方法收获制备染色体标本,R显带后用新鲜配制的1︰10姬姆萨液染色后,分析至少20个中期分裂像,计数克隆性核型异常者。[结果]358例血液系统疾病的患者中,157例检出克隆性核型异常,异常率43.9%,其中单纯数量异常者16例,占异常核型的10.2%,单纯结构异常者89例,占56.7%,数量异常与结构异常并存者52例,占33.1%。[结论]将染色体核型分析作为造血和淋巴组织肿瘤的诊断分类标准之一是非常必要的,特别对病态造血不明显的骨髓增生异常综合征(MDS)及形态学、组织细胞化学不典型的白血病具有重要的诊断、治疗和预后价值。
[Objective] To investigate the value of karyotype analysis in the diagnosis, treatment and prognosis of hematological malignancies. [Method] The bone marrow cells of 358 patients with hematological diseases were cultured for 24 hours. Then the chromosome specimens were harvested by routine methods. After R banding, the cells were stained with freshly prepared 1: 10 Giemsa stain and analyzed for at least 20 metaphase Split like, counting clonal karyotype abnormalities. [Results] Out of 358 patients with hematological diseases, 157 cases were found to have abnormal karyotype, the rate of abnormality was 43.9%. Among them, there were 16 cases with abnormal number, accounting for 10.2% of abnormal karyotype, 89 cases with simple structure abnormality, Accounting for 56.7%. The number of abnormalities and structural abnormalities coexisted in 52 cases, accounting for 33.1%. [Conclusion] Chromosomal karyotyping as one of the diagnostic criteria for hematopoietic and lymphoid tissue tumors is very necessary, especially for patients with myelodysplastic syndrome (MDS) and morphological, atypical Leukemia has important diagnostic, therapeutic and prognostic value.
