目的研究局部亚低温治疗对急性脑出血患者血清基质金属蛋白酶(MMP-9)和细胞纤维连接蛋白(cellularfib ronectin,c-Fn)的影响。方法随机将80例脑出血患者分为局部亚低温组(40例)和常规治疗组(40例),同时设健康对照组23例;常规治疗组应用药物治疗,局部亚低温组在药物治疗基础上加用局部亚低温治疗,观察两组患者发病第1、3、7d血清MMP-9和c-Fn水平的变化。结果两组患者发病第1d血清MMP-9和c-Fn水平均高于健康对照组(P<0.01);第3d常规治疗组MMP-9和c-Fn水平达高峰,局部亚低温组下降,明显低于常规治疗组(P<0.05);在发病第7d,局部亚低温组血清MMP-9和c-Fn仍明显低于常规治疗组(P<0.05)。结论颅脑局部亚低温治疗可显著降低脑出血后患者血清MMP-9和c-Fn的浓度,减轻脑出血后的炎症反应,发挥其脑保护作用。
Objective To investigate the effects of local mild hypothermia on serum levels of matrix metalloproteinase (MMP-9) and cellular fibronectin (c-Fn) in patients with acute cerebral hemorrhage. Methods 80 cases of cerebral hemorrhage were randomly divided into local mild hypothermia group (40 cases) and conventional treatment group (40 cases), while the control group of 23 patients; conventional treatment group with drug treatment, local mild hypothermia group based on drug treatment On the addition of local mild hypothermia treatment, the incidence of the first two groups were observed on the 1st, the 1st and 7th day serum MMP-9 and c-Fn level changes. Results Serum levels of MMP-9 and c-Fn on the first day after onset were higher in the two groups than those in the healthy controls (P <0.01). The levels of MMP-9 and c-Fn in the third day of treatment were significantly higher than those in the mild hypothermia group (P <0.05). Serum MMP-9 and c-Fn in the local mild hypothermia group were still significantly lower than those in the conventional treatment group on the 7th day (P <0.05). Conclusion Local mild hypothermia can significantly reduce the concentration of serum MMP-9 and c-Fn in patients with cerebral hemorrhage, relieve the inflammatory reaction after intracerebral hemorrhage, and exert its neuroprotective effect.
