目的:探讨脂联素受体2(AdipoR2)的表达变化在子痫前期发病机制中的作用。方法:收集20例正常妊娠妇女和20例子痫前期患者大网膜脂肪组织及胎盘组织,分别运用实时定量PCR技术和Western blot印迹法检测AdipoR2 mRNA及其蛋白质的表达;同时测定空腹血糖、空腹胰岛素、体重指数等临床指标。结果:①子痫前期患者存在胰岛素抵抗和高胰岛素血症。②与正常妊娠妇女比较,子痫前期患者胎盘组织中AdipoR2 mRNA及其蛋白质的表达水平均显著升高,大网膜脂肪组织中均显著降低,差异均有统计学意义(P<0.05)。③两组孕妇胎盘组织中及正常妊娠妇女大网膜脂肪组织中AdipoR2 mRNA的表达水平与BMI、FIN、FPG、HOMA-IR均无相关性(P>0.05),但子痫前期患者大网膜脂肪组织中AdipoR2 mRNA的表达水平与BMI、FIN、HOMA-IR均呈显著负相关(P<0.05)、与FPG无相关性(P>0.05)。结论:AdipoR2在胎盘组织中的表达升高,在大网膜脂肪组织中的表达降低,可能在子痫前期的发病机制中发挥重要作用。子痫前期患者大网膜脂肪组织中AdipoR2的表达下降可能是受到超重、低ADP血症、高胰岛素血症及胰岛素抵抗的影响。
Objective: To investigate the role of adiponectin receptor 2 (AdipoR2) expression in the pathogenesis of preeclampsia. Methods: The omental adipose tissue and placenta of 20 normal pregnant women and 20 preeclampsia patients were collected. The expression of AdipoR2 mRNA and protein were detected by real-time quantitative PCR and Western blotting respectively. The fasting blood glucose, fasting insulin , Body mass index and other clinical indicators. Results: ①The patients with preeclampsia had insulin resistance and hyperinsulinemia. ② Compared with normal pregnant women, the expression level of AdipoR2 mRNA and protein in placenta of preeclampsia patients were significantly increased, while in omentum adipose tissue, the difference was statistically significant (P <0.05). The expression of AdipoR2 mRNA in placenta tissue and normal pregnant women in both groups had no correlation with BMI, FIN, FPG and HOMA-IR (P> 0.05), but in the preeclampsia, AdipoR2 mRNA expression in adipose tissue was negatively correlated with BMI, FIN and HOMA-IR (P <0.05), but not with FPG (P> 0.05). Conclusions: The expression of AdipoR2 in placenta tissue is increased and its expression is decreased in omental adipose tissue, which may play an important role in the pathogenesis of preeclampsia. The decreased expression of AdipoR2 in the omental adipose tissue of preeclampsia may be due to the effects of overweight, hypointense, hyperinsulinemia and insulin resistance.
