目的:探讨产妇分娩时疼痛的影响因素及其对分娩结局的影响。方法:选取在阳泉市阳煤集团总医院住院的130例正常单胎头位产妇为研究对象,采用视觉模拟评分(VAS)法评价产妇潜伏期、活跃期的疼痛情况。同时采用Logistic回归模型分析影响分娩疼痛程度的因素,并统计产妇分娩结果。结果:所选产妇潜伏期轻度疼痛34例,中度疼痛96例。通过多因素Logistic回归分析结果显示:适龄产妇、对分娩认知不足、初产妇、夫妻感情良好是导致产妇潜伏期中度疼痛的危险因素。130例产妇中有117例进入活跃期,其中中度疼痛54例,重度疼痛63例。通过多因素Logistic回归模型分析结果显示:分娩紧张恐惧、情绪控制不良、未用镇静剂是导致产妇活跃期重度疼痛的危险因素。潜伏期中度疼痛产妇胎儿窘迫发生率、剖宫产率显著高于轻度疼痛产妇,差异有统计学意义(P<0.05);活跃期重度疼痛产妇胎儿窘迫发生率、剖宫产率、产后出血发生率显著高于中度疼痛产妇,差异有统计学意义(P<0.05)。结论:年龄较轻、初产妇、对分娩认知不足、紧张恐惧、情绪控制能力差者分娩疼痛较剧烈,而自觉疼痛程度较轻的产妇分娩结局较好。
Objective: To investigate the influencing factors of pain during childbirth and its effect on delivery outcome. Methods: 130 normal single-head mothers hospitalized in Yangquan Coal General Hospital of Yangquan were selected as research objects. Visual analogue scale (VAS) was used to evaluate the latent period and active period pain of maternal. At the same time, Logistic regression model was used to analyze the factors influencing the degree of pain during childbirth and the results of maternal childbirth were calculated. Results: 34 cases of mild pain and 96 cases of mild pain were selected as incubation period. Multivariate logistic regression analysis showed that: the appropriate age of mothers, lack of knowledge of childbirth, primiparous, good marital relationship is the risk of maternal incubation of moderate pain risk factors. 117 of 130 maternal women entered the active phase, including 54 cases of moderate pain and 63 cases of severe pain. Multivariate logistic regression model analysis showed that: labor fear of tension, emotional control is poor, without sedation is the leading cause of active maternal severe pain risk factors. The incidence of fetal distress and cesarean section rate were significantly higher in moderate-latency maternal than those in mild pain (P <0.05). The incidence of fetal distress, cesarean section rate, postpartum hemorrhage The incidence was significantly higher than moderate pain maternal, the difference was statistically significant (P <0.05). Conclusion: The younger, primipara, lack of awareness of childbirth, nervousness, fear of poor emotional control, childbirth pain more intense, and consciously less painful maternal childbirth outcomes better.
