目的探讨大鼠胃浆膜下间隙胰岛移植的可行性及其移植效果。方法应用Lewis近交系雄性大鼠,采用链脲菌素诱导糖尿病模型;同种系大鼠体外胰岛分离、纯化,将一定数量的胰岛植入糖尿病大鼠胃浆膜下间隙(胃组),同期将同等数量的胰岛植入肾被膜下间隙(肾组)作为对照,评估移植后糖尿病大鼠的血糖水平、腹腔葡萄糖耐量试验及胰岛移植物存活率。结果胰岛移植术后前3周,胃组和肾组内糖尿病大鼠的血糖水平较移植前均明显下降(P<0.05),两组大鼠间血糖水平无差异、糖耐受功能良好(P>0.05);两移植部位的胰岛被免疫组织化学染色有效确认,移植胰岛周围有少量炎性细胞浸润,肾组织样本内胰岛周围的炎细胞浸润更明显。肾组和胃组糖尿病大鼠血糖水平分别于胰岛移植术后第5、4周开始升高。术后第6周两组大鼠血糖水平均升高且糖耐量减低,免疫组织化学染色检查显示,两移植部位内仅残余散在的胰岛细胞。结论胃浆膜下间隙胰岛移植的短期效果与肾被膜下胰岛移植相同,但其血糖控制效果比肾被膜下胰岛移植更佳,而其长期效果有待进一步证实。
Objective To investigate the feasibility and the effect of transplanted islets in the subserosal space in rats. Methods Lewis inbred male rats were used streptozotocin to induce diabetes model. The same type of rats were isolated and purified from pancreatic islets in vitro. A number of islets were implanted into the subepithelial space (stomach group) of diabetic rats, In the same period, the same number of islets were implanted into the subrenal space (renal group) as a control to evaluate the blood glucose level, the intraperitoneal glucose tolerance test and the islet graft survival rate in diabetic rats after transplantation. Results In the first 3 weeks after islet transplantation, the blood glucose level of diabetic rats in gastric and renal groups were significantly lower than those before transplantation (P <0.05), there was no difference in blood glucose levels between the two groups, and glucose tolerance was good (P > 0.05). The islets of the two transplantation sites were confirmed by immunohistochemical staining. There was a small amount of inflammatory cell infiltration around the transplanted islets, and infiltration of inflammatory cells around the islets in the renal tissue samples was more obvious. Blood glucose levels in renal and gastric diabetic rats began to increase at 5 and 4 weeks after islet transplantation. At 6 weeks after operation, the blood glucose level and the impaired glucose tolerance of the two groups were decreased. Immunohistochemical staining revealed only scattered islet cells in the two transplantation sites. Conclusions The short-term effect of subepithelial islet septa islet transplantation is the same as that of subrenal islet transplantation, but its blood glucose control effect is better than that of subrenal islet transplantation, and the long-term effect remains to be confirmed.
