目的:行立体走向射频毁损杏仁海马复合体,达到消毁致痫灶,阻止癫痫放电的传播以治疗顽固性癫痫。方法:采用引进的FischerZD系统,直接在螺旋CT扫描下,认清杏仁核、海马的部位,计算三维坐标值。采用局麻,额部钻孔径路,插入直径1mm电极针至靶区,用N50型射频仪(德国产),参数75℃,90s,分别毁损杏仁、海马,制造一个约1.0cm的毁损灶。结果:本组13例,随访最长者19个月,平均10个月。得到满意结果者(癫痫发作消失几例,显著改善者(癫痫发作减少75%)7例,无效者1例。全组无并发症。提示:根据立体定向术治疗癫痫的理论依据是毁损致癌灶和破坏癫痫放电的传播径路,定位精确,手术损伤小,安全,控制癫痫效果满意。
OBJECTIVE: To stereotactic radiofrequency ablation of almond hippocampal complex to destroy the epileptogenic zone, to prevent the spread of epilepsy discharge to treat intractable epilepsy. Methods: The imported FischerZD system was used to directly identify the parts of the amygdala and hippocampus under the helical CT scan and calculate the 3D coordinates. Using local anesthesia, forehead drilling path, insert 1mm diameter electrode needle to the target area, using the N50 RF instrument (Germany), parameters 75 ℃, 90s, respectively, to destroy almonds, hippocampus, making a 1.0cm lesion . Results: The group of 13 cases, the highest follow-up of 19 months, an average of 10 months. 7 patients with marked improvement (75% reduction of seizures) and 1 ineffective (no seizures were found) .There was no complication in the whole group.It was suggested that the theory basis of stereotactic surgery for the treatment of epilepsy was that the lesions were destroyed And destroy the spread of epilepsy discharge path, accurate positioning, small surgical injury, safety, control of epilepsy with satisfactory results.
