目的:检测细胞因子IL-2、IL-6、IL-10与TNF-α在呼吸机相关性肺炎患儿支气管肺泡灌洗液(BALF)、血清中的表达水平并探讨它们对于预测判断呼吸机相关性肺炎(VAP)的临床意义。方法:60例资料完整的呼吸机相关性肺炎患儿作为VAP组,40例仅有恢复期资料的作为VAP恢复期组,60例同期健康体检的健康儿童作为对照组。利用双抗体夹心ELISA法检测两组儿童血清和支气管肺泡灌洗液中IL-2、IL-6、IL-10和TNF-α的水平。结果:VAP患儿IL-2、IL-6、IL-10和TNF-α4项指标表达水平明显高于对照组健康儿童;VAP急性期的同类资料也明显高于VAP恢复期;BALF中该4项指标的表达水平明显高于血清,两者有相关性。结论:VAP患儿血清和支气管肺泡灌洗液中IL-2、IL-6、IL-10和TNF-α的水平与VAP存在显著相关性,可为VAP的病情判断提供分子标志物,并为深入探讨VAP的发病机制提供科学依据。
Objective: To investigate the expression of IL-2, IL-6, IL-10 and TNF-α in bronchoalveolar lavage fluid (BALF) and serum in children with ventilator-associated pneumonia Clinical significance of related pneumonia (VAP). Methods: Sixty children with ventilator-associated pneumonia with complete data were selected as the VAP group, 40 patients with convalescence only as the VAP convalescent group, and 60 healthy children with the same period of physical examination as the control group. The levels of IL-2, IL-6, IL-10 and TNF-α in serum and bronchoalveolar lavage fluid of two groups of children were detected by double antibody sandwich ELISA. Results: The levels of IL-2, IL-6, IL-10 and TNF-α in children with VAP were significantly higher than those in healthy children in the control group. Similar data in the acute phase of VAP were also significantly higher than those in VAP recovery. In BALF, The level of expression of the index was significantly higher than the serum, the two are related. CONCLUSION: The levels of IL-2, IL-6, IL-10 and TNF-α in serum and bronchoalveolar lavage fluid of children with VAP are significantly correlated with VAP, which can provide molecular markers for the judgment of VAP and are In-depth discussion of the pathogenesis of VAP provides a scientific basis.
