目的　为了在Halsted乳癌根治术中达到既根治又美容的目的 ,我们对 2 1例乳癌根治手术进行改良。方法　改良措施包括 :改纵行切口为斜行的Orr切口 ;采用自制的二齿皮肤牵拉钳和双刃刀均匀游离皮瓣 ;游离带蒂背阔肌肌瓣 ,填充于腋窝 ;创面喷洒ZT胶或生物蛋白胶 ;将皮瓣与肋间肌缝合 ;腋窝低压负压持续吸引 ;以可吸收薇乔线做皮内缝合。结果　 2 1例患者中术后发生皮瓣坏死的 1例 ,无一例发生腋窝积液 ,1例发生切口旁皮下少量积液 ,所有患者平均住院 12天。结论　通过上述改良措施 ,既根治彻底 ,又减少了腋窝积液 ,皮瓣坏死等并发症 ,保持了外形的美观 ,提高了患者术后的生活质量 Objective In order to achieve the goals of radical cure and beauty treatment in Halsted radical mastectomy, we improved 21 radical operations for breast cancer. Methods Improvement measures include: Orig incision with vertical incision oblique incision; self-made two-tooth skin pulling forceps and double-edged knife uniform free flap; free pedicle latissimus dorsi muscle flap, filling in axillary fossa; wound spray ZT Gel or bioprotein glue; the skin flap and intercostal muscle suture; low pressure negative pressure in the popliteal fossa continues to attract; intradermal suture with absorbable Vichy line. Results One case of flap necrosis was found in 21 patients. There was no case of popliteal effusion, and 1 patient had a small amount of subcutaneous effusion beside the incision. All patients were hospitalized for an average of 12 days. Conclusion Through the above-mentioned improvement measures, it can not only radically cure, but also reduce the complications of effusion, flap necrosis and other complications, maintain the appearance of the appearance, improve the quality of life of patients after surgery.
