目的探讨经颅彩色超声检查(TCCS)评估神经危重症患者脑中线移位(MLS)的可靠性及可行性。方法 70例行头颅计算机X射线断层扫描(CT)检查的患者,根据中线结构是否居中,分为居中组和移位组,分别在3h内完成床旁TCCS检查,测得第三脑室MLS值,分析两种检查方法的相关性。结果在神经危重症患者中TCCS和头颅CT测得中线移位结果高度一致,居中组TCCS测得MLS值为(-0.01±1.50)mm,两种方法所测MLS的差值为(0.12±0.71)mm,两者无统计学差异。移位组两种方法测得MLS的相关系数为0.981(P<0.001)。结论 TCCS检查是一种无创、便携、可重复且可靠的床旁监测第三脑室中线移位的技术,与头颅CT具有良好的相关性。
Objective To investigate the reliability and feasibility of transcranial color sonography (TCCS) in assessing the midline shift (MLS) in patients with neurological crisis. Methods 70 patients undergoing cranial computed tomography (CT) examination were divided into centering group and transposition group according to whether the midline structure was centered or not. TCCS was performed within 3h, and the third ventricular MLS value was measured. Analyze the correlation between the two methods of examination. Results The results of TCCS and cranial computed tomography showed that the midline shift was highly consistent in patients with critical neurological disease. The median MLS measured by TCCS was (-0.01 ± 1.50) mm in the middle group, and the difference between the two methods was (0.12 ± 0.71) ) mm, no significant difference between the two. The correlation coefficient of the MLS measured by the two methods in the shift group was 0.981 (P <0.001). Conclusion TCCS is a non-invasive, portable, repeatable and reliable bedside monitoring of the third ventricle midline shift technology, and has a good correlation with the cranial CT.
