目的初步研究伽玛刀治疗囊性听神经瘤的效果及并发症。方法我科从1997年到2010年应用OUR-旋转式伽玛刀治疗囊性听神经瘤共12例,其中男性7例,女性5例,年龄43～61岁,平均53岁。所有患者均未行手术或活检,临床及影像诊断为听神经瘤。肿瘤直径为3mm～4.2cm,所有患者无颅内高压,无明显脑积水;肿瘤部位在右侧者有7例,左侧者有5例,治疗周边剂量8～14Gy,等剂量曲线为50%。结果在随访期间,肿瘤缩小者有7例(87.5%),其中1例(12.5%)患者于治疗后6个月影像学显示肿瘤体积增大,考虑多系肿瘤坏死水肿所致,5年后随访提示缩小;肿瘤无增大者为1例。临床症状改善有6例,无变化有2例。结论在严格制适应证的前提下,伽玛刀治疗囊性听神经瘤疗效肯定,并发症少。
Objective To study the efficacy and complications of gamma knife in the treatment of cystic acoustic neuroma. Methods From 1997 to 2010, our department applied OUR-rotary gamma knife to treat cystic acoustic neuroma in 12 cases, including 7 males and 5 females, aged from 43 to 61 years old with an average of 53 years. All patients did not undergo surgery or biopsy, clinical and imaging diagnosis of acoustic neuroma. Tumor diameter was 3mm ~ 4.2cm, all patients had no intracranial hypertension, no significant hydrocephalus; tumor site on the right side in 7 cases, the left side in 5 cases, the treatment of peripheral dose of 8 ~ 14Gy, isodose curve of 50 %. Results During the follow-up period, 7 cases (87.5%) had tumor shrinkage. One case (12.5%) showed tumor volume increase 6 months after the treatment. Considering multi-line tumor necrosis edema, after 5 years Follow-up prompts to narrow; no increase in tumor was 1 case. Clinical symptoms improved in 6 cases, no change in 2 cases. Conclusion In strict accordance with the indications of the premise, gamma knife treatment of cystic acoustic neuroma sure, less complications.
