目的通过比较64层3D-CTA与DSA在临床诊断颅内动脉瘤及其评估外科治疗的优缺点,评价64层3D-CTA在颅内动脉瘤诊断及治疗中的临床应用价值。方法回顾性分析2005年9月至2006年12月期间,重庆医科大学附属第一医院神经外科99例自发性蛛网膜下腔出血患者的临床资料,患者均行3D-CTA检查,分析CTA检查结果并与DSA检查结果比较。结果99例SAH患者经3D-CTA检查发现动脉瘤70例,其中41例同时接受DSA检查,除1例外,全部证实DSA的发现, DSA所漏诊的1例经手术证实为微小动脉瘤。结论64层螺旋CTA对颅内动脉瘤的诊断敏感性及特异性高,且能够清楚显示解剖结构,有助于治疗方法的选择和难度的评估。
Objective To evaluate the clinical value of 64-dimensional 3D-CTA in the diagnosis and treatment of intracranial aneurysms by comparing 64-slice 3D-CTA with DSA in the diagnosis of intracranial aneurysms and evaluating the advantages and disadvantages of surgical treatment. Methods The clinical data of 99 patients with spontaneous subarachnoid hemorrhage in the Department of Neurosurgery of the First Affiliated Hospital of Chongqing Medical University from September 2005 to December 2006 were analyzed retrospectively. The patients underwent 3D-CTA and analyzed the results of CTA And compared with the DSA test results. Results A total of 70 aneurysms were detected in 99 SAH patients by 3D-CTA. Among them, 41 patients underwent DSA at the same time. All 1 patients were confirmed DSA. One patient missed by DSA was proved to be a small aneurysm by operation. Conclusion 64-slice CTA has high sensitivity and specificity for the diagnosis of intracranial aneurysms, and can clearly show the anatomic structure, which is helpful for the selection of treatment methods and evaluation of the difficulty.
