目的探讨重症急性胰腺炎(SAP)致多器官功能不全综合征(MODS)的临床病理过程及发病机制。方法回顾性分析92例SAP及其中28例并发MODS者的临床资料。结果本组28例MODS的发生与重度胰腺坏死、早期循环休克及继发性胰腺感染等因素有关。死亡30例,死亡率32.61%。结论发生于胰腺炎病程不同时期,临床经过也不一致的MODS病例,却以显著的全身脓毒性反应为共同特征,SAP后MODS是由局部胰腺毒性病灶始动而波及全身各器官系统的炎症性病理过程。
Objective To investigate the clinicopathological process and pathogenesis of multiple organ dysfunction syndrome (MODS) caused by severe acute pancreatitis (SAP). Methods The clinical data of 92 patients with SAP and 28 patients with MODS were retrospectively analyzed. Results The incidence of MODS in 28 cases was related to severe pancreatic necrosis, early circulatory shock and secondary pancreatic infection. 30 cases died, the mortality rate was 32.61%. Conclusions The occurrence of pancreatitis in different periods of disease, the clinical experience is not consistent with the MODS cases, but with significant systemic septic reactions as a common feature, SAP after the onset of pancreatic pancreatic lesions began to affect the systemic inflammatory pathology process.
