目的:探讨微血管减压术治疗三叉神经痛的临床效果及相关因素进行分析。方法:对经CT、MRI排除器质性病变的239例三叉神经痛病人采用微血管减压术进行治疗,并在术中对血管与三叉神经的关系进行观察确定责任血管后进行处理。结果:血管与三叉神经的关系可分为4种类型即:接触型、袢状压迫型、黏连型和穿通型,其手术效果以接触型与袢状压迫型最好,而黏连型与穿通型稍差。结论:微血管减压术治疗三叉神经痛的效果肯定,其效果与血管和三又神经的关系密切相关,精良显微器械和娴熟的手术技巧是提高疗效和减少并发症的关键。
Objective: To investigate the clinical effect and related factors of microvascular decompression in the treatment of trigeminal neuralgia. Methods: A total of 239 patients with trigeminal neuralgia who had been excluded by CT and MRI were treated with microvascular decompression. The relationship between vascular and trigeminal nerve was observed during surgery to determine the responsible vessel. Results: The relationship between blood vessels and the trigeminal nerve could be divided into four types: contact type, pusiform compression type, adhesion type and penetrating type. The surgical effect was the best with contact type and plicate compression type, while the adhesion type and Through type slightly worse. Conclusion: The effect of microvascular decompression for the treatment of trigeminal neuralgia is affirmed. The effect is closely related to the relationship between blood vessels and the three nerves. The excellent micro-instruments and skilled surgical techniques are the keys to improve curative effect and reduce complications.
