目的:揭示MUC3粘蛋白在人正常胆囊、结石性胆囊和胆囊息肉黏膜组织中粘蛋白的表达规律和作用.方法:用抗MUC3粘蛋白的单克隆抗体及免疫组化方法检测上述组织中粘蛋白的表达.结果:结石性胆囊(胆固醇结石、胆红素结石)和腺瘤性息肉胆囊组织中MUC3粘蛋白的表达阳性率分别为29.6%(16/54),21%(8/38),8.33%(2/24),较对照组75%(18/24)有显著性差异.MUC3粘蛋白在对照组胆囊分布于柱状细胞的胞质内,呈颗粒状分布,两极密度较核周大,细胞核不表达,腺体细胞表面也有厚薄不均的一层.结石性胆囊粘蛋白表达阳性率低于对照组,密度也降低,分布与后者基本相同,有的标本在胞质内呈均匀分布.腺瘤息肉胆囊MUC3粘蛋白的表达阳性率最低,表达阳性者主要在柱状细胞,杯状细胞也有表达,也呈颗粒状分布,细胞核内不表达.结论:MUC3粘蛋白在正常胆囊组织的存在较普遍;在正常黏膜向炎症和化生/不典型增生转化过程中是下调表达的,提示黏膜的异常转化涉及粘蛋白基因(也可能是癌基因)的抑制或丢失.
