目的了解深圳市男男性接触者(MSM)商业性性行为的特征及其影响因素,为制定防治策略提供参考依据。方法通过自愿咨询检测和场所调查相结合的方法,于2008年6~9月在深圳市对MSM进行问卷调查,调查内容包括社会人口学和高危性行为特征。结果调查的455名MSM中,22.2%的人在近6个月内发生过商业性性行为,其中仅向男性提供商业性行为和为两性均提供商业性性行为的分别占77.2%和21.8%,仅有1人只向女性提供商业性行为。最近一次与男性(肛交)和女性发生商业性性行为时使用安全套率分别为92.0%和78.3%,二者差别无统计学意义(χ2=2.42,P=0.120)。在发生商业性性行为的101名MSM中,HIV和梅毒的感染率分别为2.97%和7.92%,非商业性性行为的352名MSM中,HIV和梅毒的感染率分别为5.40%和13.35%,HIV、梅毒感染率在两类人群中的差别均无统计学意义(P>0.05)。多因素logistic回归分析显示,年龄≤26岁(OR,0.112;95%CI,0.026~0.489)、全职(OR,2.811;95%CI,1.168~6.674)分别为MSM人群近6个月发生商业性性行为的危险因素和保护因素。结论MSM人群中发生商业性性行为的比例较高,年龄和职业对此行为有关。
Objective To understand the characteristics and influencing factors of commercial sexual behavior of MSM in Shenzhen and provide reference for formulating prevention and control strategies. Methods Through a combination of voluntary counseling and testing and site investigation, a questionnaire survey was conducted in Shenzhen from June to September in 2008. The survey included social demography and high-risk sexual behaviors. Results Of the 455 MSMs surveyed, 22.2% of them had had commercial sexual behaviors within the past 6 months, of which 77.2% and 21.8% respectively provided commercial sex only to men and commercial sex both sexes. Only one person provides commercial sexual activity to women only. The most recent condom use rate for commercial sex with men (anal sex) and women was 92.0% and 78.3%, respectively, with no significant difference (χ2 = 2.42, P = 0.120). The prevalence of HIV and syphilis in 101 MSMs with commercial sexual behaviors was 2.97% and 7.92% respectively, and among 352 MSMs without commercial sex, the prevalences of HIV and syphilis were 5.40% and 13.35%, respectively There was no significant difference in HIV and syphilis infection rates between the two groups (P> 0.05). Multivariate logistic regression analysis showed that commercialization occurred in the MSM population for nearly 6 months with age ≤26 years (OR, 0.112; 95% CI, 0.026-0.489), full-time (OR, 2.811; 95% CI, 1.168-6.674) Risk factors and protective factors of sexual behavior. Conclusion The proportion of MSM in commercial sexual behavior is high, while age and occupation are related to this behavior.
