背景:不稳定膀胱的发生与毒蕈碱受体(MR)变化有关,MR有5种亚型,而膀胱逼尿肌仅有两种亚型—M2R,M3R,目前尚不清楚MR亚型在不稳定膀胱发生中的作用。目的:探讨MR亚型密度变化与不稳定膀胱发生之间的关系。设计:随机对照的实验研究。单位:解放军第三军医大学西南医院泌尿中心。材料:实验在重庆西南医院中心实验室完成,选用为30只Wistar大鼠。方法:建立不稳定膀胱大鼠模型,将30只大鼠分成实验组和对照组各15只,实验组在10g/L苯巴比妥麻醉下,对大鼠施行尿道结扎手术,造成尿道不完全梗阻。对照组大鼠仅切开皮肤,而不结扎尿道。采用RT-PCR方法测定实验组大鼠不稳定膀胱及对照组逼尿肌MR亚型密度。主要观察指标:尿动力学,M2RNA和M3RNA含量,及两者的比值变化。结果:对照组逼尿肌组织含有M2RNA和M3RNA,M2RNA(1.67±0.42)含量明显多于M3RNA(0.64±0.19),两者之比约2.59∶1,没有检测到M1,M2和M3亚型的mRNA。实验组大鼠不稳定膀胱逼尿肌中M2和M3密度均增加,分别为(2.03±0.65)和(1.53±0.46),M3增加更明显(P<0.01),两者比值显著缩小,约1.4∶1。结论:不稳定膀胱产生与MR亚型上调及M2R,M3R亚型之间的比例失调有关。
BACKGROUND: The occurrence of unstable bladder is related to the changes of muscarinic receptors (MR). There are 5 subtypes of MR, but there are only 2 subtypes of bladder detrusor - M2R and M3R. It is not clear that MR subtype The role of unstable bladder. Objective: To investigate the relationship between MR subtype density changes and unstable bladder. Design: Randomized controlled experimental study. Unit: Southwest Hospital, Third Military Medical University, Urology Center. MATERIALS: The experiment was performed in the Central Laboratory of Southwest Hospital of Chongqing and selected as 30 Wistar rats. Methods: The rat model of unstable bladder was established. 30 rats were divided into experimental group and control group, 15 rats in each group. Under the anesthesia of 10g / L phenobarbital, urethral ligation was performed in rats, resulting in incomplete urethral obstruction. Control rats only cut the skin without ligation of the urethra. The densities of detrusor MR subtype in the unstable bladder of the experimental group and the control group were determined by RT-PCR. MAIN OUTCOME MEASURES: Urodynamics, M2RNA and M3RNA contents, and their ratio changes. Results: Compared with M3RNA (0.64 ± 0.19), M2RNA and M3RNA in detrusor tissue of control group were significantly higher than that of M3RNA (0.64 ± 0.19), the ratio of the two was about 2.59:1. There were no detectable M1, M2 and M3 subtypes mRNA. The densities of M2 and M3 in the detrusor instability of rats in the experimental group were increased (2.03 ± 0.65) and (1.53 ± 0.46) respectively, and the increase of M3 was more obvious (P <0.01), the ratio of the two was significantly reduced, about 1.4 :1. CONCLUSION: Unstable bladder production is associated with upregulation of MR subtypes and imbalance of M2R and M3R subtypes.
