目的应用定量组织速度成像评价无明显症状酗酒者的左室舒张功能。方法酗酒组31例及对照组30例。常规超声心动图测量二尖瓣口血流快速充盈速度(E),左房收缩期速度(A),计算E/A。应用实时三平面定量组织速度成像技术测量后间隔、左室侧壁、前壁、下壁、前间隔和后壁的二尖瓣环、基底段、中间段水平舒张早期峰值速度(Ve)、左房收缩期峰值速度(Va)、计算Ve/Va。并比较二组间各参数。结果酗酒者二尖瓣水平及大部分节段Ve,Ve/Va减低,Va增高(P<0.05);定量组织速度成像较脉冲多普勒对酗酒者早期左心室舒张功能异常的检出率更高(P<0.05)。结论无症状酗酒者早期即存在左心功能的异常,应用定量组织速度成像评价酗酒者左心舒张功能异常较传统超声更敏感,更可靠。
Objective To evaluate left ventricular diastolic function in patients with alcoholism without obvious symptoms by quantitative tissue velocity imaging. Method 31 cases of alcoholism group and 30 cases of control group. Conventional echocardiography was performed to measure mitral inflow velocity (E), left atrial systolic velocity (A) and E / A. The mitral annular velocity, basal segment and middle segment of early posterior wall, anterior wall, anterior wall, inferior wall, anteroseptal and posterior wall were measured by real-time three-plane quantitative tissue velocity imaging technique. Peak early diastolic velocity (Ve) Systolic peak systolic velocity (Va), calculated Ve / Va. And compare the parameters between the two groups. Results The level of mitral valve and the decrease of Ve, Ve / Va and Va in alcoholism group were significantly higher than those in alcoholic group (P <0.05). Compared with pulse Doppler, quantitative tissue velocity imaging showed more detection rate of left ventricular diastolic dysfunction in alcoholics High (P <0.05). Conclusion Asymptomatic alcoholics have abnormalities of left ventricular function early. It is more sensitive and reliable to evaluate left ventricular diastolic dysfunction in alcoholics by quantitative tissue velocity imaging.
