目的探讨慢性阻塞性肺病(chronic obstructive pulmonary disease,COPD)患者支气管肺泡灌洗液(bronchoalveolar lavage fuid,BALF)中细胞计数和IL-8变化。方法选择本科住院的30例COPD急性发作期患者为A组,稳定期患者30例为B组;健康组8例为C组。对3组分别进行肺功能测定及BALF,BALF处理后作细胞学分析,并检测IL-8水平。结果 COPD组FEV1、FEV1/FVC低于健康组,A组低于B组,差异均有显著性(P<0.01);A、B、C组BALF白细胞总数无显著性差异(P>0.05);而A组BALF中性粒细胞比例和IL-8水平均高于B组,B组高于C组,3组两两比较,差异均有统计学意义(P<0.01);IL-8浓度与肺功能呈负相关(r=-0.792,P<0.01),与PMN呈正相关(r=0.862,P<0.01)。结论 BALF中性粒细胞比例和IL-8水平能反映COPD气道炎症情况。
Objective To investigate the changes of cell count and IL-8 in bronchoalveolar lavage fluid (BALF) in patients with chronic obstructive pulmonary disease (COPD). Methods Thirty cases of acute exacerbation of chronic obstructive pulmonary disease were enrolled in this study. The patients in group A were stable in group A, and the group B in stable group was 30 in group B. The other 8 in group C were healthy. The lung function of BALF and BALF in 3 groups were analyzed by cytology and the level of IL-8 was detected. Results The FEV1 and FEV1 / FVC in COPD group were lower than those in healthy group, while those in group A were lower than those in group B (P <0.01). There was no significant difference in BALF leucocyte count between groups A, B and C (P> 0.05). The BALF neutrophil ratio and IL-8 level in group A were higher than those in group B, while those in group B were higher than those in group C (P <0.01). The levels of IL-8 and IL- Lung function was negatively correlated (r = -0.792, P <0.01), and positively correlated with PMN (r = 0.862, P <0.01). Conclusion BALF neutrophil ratio and IL-8 level can reflect the airway inflammation in COPD.
