目的探讨HPV E6/E7 mRNA检测在30岁以下年轻女性宫颈癌筛查及宫颈疾病诊治中的应用效果,为临床筛查措施的实施提供理论依据。方法选取2013年1月-2015年1月东莞市大朗医院收的30岁以下女性宫颈液基细胞学检查结果为阳性的标本310例为研究对象。对这些标本行TBS分级和HPV E6/E7 mRNA检测。另外,对其中156例受试者同时接受病理学检查。对于宫颈液基细胞学检查结果阳性的标本行HPV E6/E7 mRNA检测。记录宫颈液基细胞学TBS分级情况、病理学分级情况和标本的HPV E6/E7 mRNA检测结果,并采用spearman相关性分析分析宫颈液基细胞学TBS分级、病理学分级和HPV E6/E7mRNA检测结果的相关性。结果在TBS分级方面,ASC-H的HPV E6/E7 mRNA阳性率最高,为80.00%,LSIL和HSIL的HPV E6/E7 mRNA阳性率分别为78.38%和71.11%。Spearman相关性分析结果显示,TBS分级与HPV E6/E7 mRNA阳性率、HPV E6/E7 mRNA拷贝数均有相关性(r=0.501、0.529,均P<0.05);在病理学分级方面,CINⅢ的HPV E6/E7 mRNA阳性率最高,为81.25%,其次为CINⅡ,阳性率为79.69%。spearman相关性分析结果显示,病理学分级与HPV E6/E7 mRNA阳性率、HPV E6/E7 mRNA拷贝数均有相关性(r=0.442、0.538,均P<0.05)。结论 HPV E6/E7 mRNA的阳性率和拷贝数可作为预测宫颈病变的良好指标。定期接受HPV E6/E7 mRNA检测是30岁以下年轻女性宫颈癌筛查及发现宫颈疾病的重要措施。
Objective To investigate the application of HPV E6 / E7 mRNA in the screening of cervical cancer and diagnosis and treatment of cervical cancer in young women under 30 years of age, and to provide a theoretical basis for the implementation of clinical screening measures. Methods From January 2013 to January 2015, Dongguan Dalang Hospital received 30 cases of female cervical cytology-positive cytology test specimens of 310 cases for the study. TBS grading and HPV E6 / E7 mRNA detection were performed on these specimens. In addition, 156 of the subjects received pathological examination simultaneously. HPV E6 / E7 mRNA was detected in cervical smear-positive test specimens. Regular HPV E6 / E7 mRNA testing is an important measure for cervical cancer screening and cervical disease in young women under 30 years of age.
