目的 :探讨抗心磷脂抗体(ACA)和抗精子抗体(As Ab)在女性不孕诊断中的价值。方法 :选取2012年1~12月湖南省妇幼保健院确诊为不孕症的200例女性患者及健康体检者100例,采用酶联免疫吸附实验(ELISA)检测2组血清抗精子抗体(As Ab)、抗心磷脂抗体(ACA)并比较不孕症组与对照组2项抗体的阳性率。结果 :血清2项生殖免疫性抗体阳性者79例,阳性率39.5%;不孕患者组As Ab阳性率25.5%(51/200)与健康对照组As Ab阳性率1%(1/100)比较,差异有统计学意义(P=0.000);不孕患者组ACA阳性率为24%(48/200)与健康对照组ACA阳性率1%(1/100)比较,差异有统计学意义(P=0.0000)。As Ab-Ig G,As Ab-Ig M,As Ab-Ig A和ACA-Ig M、ACA-Ig C阳性率与对照组比较差异均有统计学意义(P<0.05);ACA-Ig A阳性率与对照组比较差异无统计学意义(P>0.05);As Ab-Ig M和ACA-Ig G在不孕不育组内与其他类型抗体阳性率比较,差异均有统计学意义(P<0.05)。As Ab、ACA阳性率在不同年龄段间与对照组比较,3组差异均有统计学意义(P<0.05)。结论 :As Ab、ACA是引起不孕不育的重要的免疫因素,且As Ab-Ig M和ACA-Ig G与女性不孕不育的关系更加紧密。这些抗体对不同年龄人群的不孕不育患者均有影响,因此As Ab、ACA的检测对免疫性因素引起不孕不育患者的诊断和治疗具有临床价值。
Objective: To investigate the value of anticardiolipin antibody (ACA) and antisperm antibody (As Ab) in the diagnosis of female infertility. Methods: A total of 200 female patients diagnosed as infertility in Hunan MCH and 100 healthy volunteers from January 2012 to December 2012 were enrolled in this study. Serum anti-sperm antibody (As Ab) was detected by enzyme linked immunosorbent assay (ELISA) ), Anti-cardiolipin antibody (ACA) and compare the positive rate of two antibodies in infertility group and control group. Results: 79 cases were seropositive with two antibodies of reproductive immunity, the positive rate was 39.5%. The positive rate of As Ab in infertile patients was 25.5% (51/200) compared with 1% (1/100) in healthy controls (P = 0.000). The positive rate of ACA was 24% (48/200) in infertility patients and 1% (1/100) in healthy controls, the difference was statistically significant (P = 0.0000). The positive rates of As Ab-Ig G, As Ab-Ig M, As Ab-Ig A and ACA-Ig M, ACA-Ig C were significantly different from those of the control group (P <0.05) (P> 0.05). The positive rates of As Ab-Ig M and ACA-Ig G in infertility group were significantly higher than those in other groups (P < 0.05). As Ab, ACA positive rate in different age groups compared with the control group, the three groups were statistically significant differences (P <0.05). CONCLUSION: As Ab and ACA are important immune factors in causing infertility, and As Ab-Ig M and ACA-Ig G are more closely related to female infertility. These antibodies have an impact on infertility patients of different ages, so the detection of As Ab and ACA has clinical value in the diagnosis and treatment of infertility caused by immune factors.
