目的探讨埋藏电极后皮质电刺激在儿童癫疒间灶切除术功能区定位的应用。方法回顾性分析10例考虑致疒间灶累及功能区皮质的病儿资料,先行硬膜下皮质电极埋藏再行皮质电刺激以定位功能区。手术前后根据Karnofsky评分进行功能评估。结果 10例病儿均能准确定位功能区,刺激过程无明显相关并发症出现。随访神经功能均保护良好,癫疒间控制达Engle分级Ⅰ级7例,Ⅱ级2例,Ⅲ级1例。结论对致疒间灶邻近功能区的癫疒间病儿,行颅内电极埋藏后再行皮质电刺激可在术前明确皮质功能区位置,定位准确,精确指导术中保护功能区。
Objective To investigate the application of buried electrode posterior cortex electrical stimulation in children with functional epileptic resection. Methods A retrospective analysis of 10 cases of children involved in the foci involving the cortex of the functional area of the cortex children data, the first subdural cortical electrodes buried cortical electrical stimulation and then to locate the functional area. Functional assessment was based on Karnofsky score before and after surgery. Results 10 cases of sick children can accurately locate the functional area, the stimulation process without obvious complications. Neurological function was well protected during follow-up. There were 7 cases of grade Ⅰ Engle in grade control, 2 cases of grade Ⅱ and 1 case of grade Ⅲ in epilepsy. Conclusions For children with epilepsy in adjacent functional areas, the cortical electrical stimulation after burying the intracranial electrodes can be used to determine the location of the cortical functional areas before surgery. The accurate location and accurate guidance can guide the intraoperative protection of functional areas.
