目的探讨肝癌的超声征象、组织病理与微血管密度(MVD)、细胞增殖核抗原(PCNA)表达间的关系。方法将41例在超声引导下穿刺活检经病理证实或经手术切除经病理证实的肝癌,术前全部常规行二维及彩色多普勒超声检查,并按组织学分级(Edmondson分级法)分类,全部病例均行MVD及PCNA免疫组化染色。结果①MVD、PCNA与肿瘤的分化程度、大小、包膜状况、肿瘤坏死及门静脉癌栓形成呈密切相关性,差异有统计学意义。②MVD、PCNA与肿瘤转移也存在密切相关,差异有统计学意义。③MVD、PCNA两指标呈正相关趋势。结论肝癌的超声检测结果与病理评价指标具有一致性,证实了超声技术在肝癌的诊断及估测肝癌的发展及预后发挥了重要作用。
Objective To investigate the relationship between ultrasonic signs of liver cancer, histopathology and microvessel density (MVD) and cell proliferating nuclear antigen (PCNA) expression. Methods Totally 41 cases of liver cancer confirmed by pathology or surgically resected by biopsy under ultrasound guidance were examined by two-dimensional and color Doppler sonography before operation. All patients were classified by histological grading (Edmondson grading) All cases underwent MVD and PCNA immunohistochemical staining. Results ①MVD, PCNA and tumor differentiation degree, size, capsule status, tumor necrosis and portal vein thrombosis were closely related, the difference was statistically significant. ②MVD, PCNA and tumor metastasis are also closely related, the difference was statistically significant. ③MVD, PCNA two indicators showed a positive correlation trend. Conclusions The results of Hypertrophy and Histopathological evaluation of HCC are in agreement. It confirms that Ultrasound plays an important role in the diagnosis of HCC and the estimation of the development and prognosis of HCC.
