[目的]评估TKA术后患肢不同体位对术后隐性出血量和活动度的影响.[方法]计算机检索Conchrane Library、PubMed、Embase、OVID、CBM、CNKI、万方和维普数据库中关于TKA术后患肢体位对术后隐性出血量影响的随机对照试验(RCT),检索时限为建库至2016年5月,同时查阅所获取的参考文献索引.两名研究者对检索收集到的文献按照制定的纳入排除标准进行独立筛选.采用隐性出血量、总失血量、输血率、术后血红蛋白水平、术后活动度、发生深静脉血栓率作为评价指标.对纳入文献进行方法学质量评价,采用Review Manager 5.3软件进行Meta分析.[结果]最终纳入13篇随机对照研究RCT,1 775例患者.Meta分析结果显示:两组患者在隐性出血量、总失血量、输血率、术后血红蛋白水平、关节活动度差异均有统计学意义;深静脉血栓发生率差异无统计学意义.[结论]患肢屈曲位作为有效且安全的方法对TKA术后减少隐性失血量、总失血量、术后血红蛋白降低量、输血率和增加关节活动度有显著帮助.“,”[Objective] To evaluate the influence of postoperative leg flexion position on hidden blood loss and range of motion (ROM) after TKA.[Method] A randomized controlled trials (RCTs) analysis was carried out to compare the effect of flexion position and extension position on hidden blood loss and ROM after TKA.Different electronic databases were searched,including Cochrane library,Embase,PubMed,OVID,Chinese Biological Medicine,VIP,Wanfang and China National Knowledge Infrastructure database.The time limit was from the establishing of these database to May 2016.Methodology was applied for quality evaluation and meanwhile,the Review Manager 5.3 software was used for Meta-analysis.[Result] Thirteen RCTs were included in RCT with 1775 patients in all.There were significant differences in the hidden blood loss (MD=-116.08,95% CI [-125.40,-106.77],P<0.001),total blood loss (mean difference MD=-154.83,95% CI [-185.32,-124.34],P<0.00001),blood transfusion requirement (risk difference [RD] RD=-0.10,95% CI [-0.19,-0.02],P=0.002),postoperative hemoglobin level (MD=0.73,95% CI [0.42,1.04],P<0.00001) and range of motion (MD=3.15,95% CI [1.58,4.71],P<0.00001) between the flexion group and extension group.No significant differences was found regarding deep vein thrombosis (DVT) between the two groups.[Conclusion] The postoperative flexion position of the leg after TKA is effective and safe,significantly decreasing hidden blood loss,total blood loss and blood transfusion requirement.In addition,the postoperative range of motion is significantly improved by the flexion position of the leg.
