目的:探讨乙型肝炎病毒(hepatitis B virus,HBV)感染患者外周血中性粒细胞与淋巴细胞比值(neutrophil-to-lymphocyte ratio,NLR)、白介素-6(inter-leukin-6,IL-6)表达与肝功能指标谷丙转氨酶(alanine transaminase,ALT)、谷草转氨酶(aspartate aminotransferase,AST)和谷氨酰氨基转移酶(glutamyl transferase,GGT)的关系。方法:选取我院2018年1月至2019年8月收治的HBV感染患者,以HBV-DNA载量分为低拷贝组、中拷贝组和高拷贝组,根据Child-pugh肝功能分级分为A级、B+C级患者。检测对比3组及不同肝功能分级患者血清IL-6、NLR水平,分析血清IL-6、NLR水平与HBV-DNA载量、肝功能指标(ALT、AST和GGT)相关性,及HBV感染患者肝功能的影响因素。结果:共纳入136例患者作为研究对象,低拷贝组42例、中拷贝组61例和高拷贝组33例。高拷贝组血清IL-6、NLR水平分别为(28.39±6.11)ng/L、(7.26±1.48),高于中拷贝组的(21.62±5.23)ng/L、(5.14±1.21)及低拷贝组(16.13±4.05)ng/L、(3.51±1.06)(n F=51.113,n P<0.05;n F=83.217,n P<0.05);血清IL-6(n r=0.551)、NLR(n r=0.670)与HBV-DNA载量呈正相关(n P<0.05);血清IL-6(n r1=0.556;n r2=0.529;n r3=0.503)、NLR(n r1=0.615;n r2=0.598;n r3=0.603)与ALT、AST、GGT呈正相关(n P<0.05);HBV-DNA载量(n OR=4.088)、病程(n OR=2.848)、血清IL-6(n OR=3.843)和NLR(n OR=3.562)是HBV感染患者肝功能的重要影响因素(n P<0.05)。n 结论:HBV感染患者外周血NLR、IL-6呈高表达,与HBV-DNA载量、肝功能相关。“,”Objective:To investigate the relationship between the expression of peripheral blood neutrophil-to-lymphocyte ratio (NLR) and inter-leukin-6 (IL) and liver function indicators, alanine transaminase (ALT), aspartate aminotransferase (AST) and glutamyl transferase (GGT) in patients with hepatitis B virus (HBV) infection.Methods:HBV-infected patients admitted in our hospital from January 2018 to August 2019 were selected and divided into low copy group, middle copy group and high copy group based on HBV-DNA load. The patients were divided into patients of grade A and grade B+C according to Child-pugh liver function classification. The serum IL-6 and NLR levels in three groups and patients with different liver function grades were detected and compared. The correlations among serum IL-6 and NLR levels with HBV-DNA load, liver function indicators (ALT, AST, GGT) as well as the influencing factors of liver function in HBV-infected patients were analyzed.Results:A total of 136 patients were included in the study, including 42 in the low copy group, 61 in the middle copy group, and 33 in the high copy group. The serum levels of IL-6 and NLR in the high copy group were (28.39±6.11) ng/L and (7.26±1.48). These were higher than the levels of (21.62±5.23) ng/L and (5.14±1.21) in the middle-copy group as well as the levels of (16.13±4.05) ng/L and (3.51±1.06) low-copy group (n F=51.113, n P<0.05;n F=83.217, n P<0.05). Serum IL-6 (n r=0.551) and NLR (n r=0.670) were positively correlated with HBV-DNA load (n P<0.05), respectively. Serum IL-6 (n r1=0.556; n r2=0.529; n r3=0.503) and NLR (n r1=0.615; n r2=0.598; n r3=0.603) were positively correlated with ALT, AST, and GGT (n P<0.05), respectively. HBV-DNA load (n OR=4.088), course of disease (n OR=2.848), serum IL-6 (n OR=3.843) and NLR (n OR=3.562) were important influencing factors of liver function in HBV-infected patients (n P<0.05).n Conclusions:NLR and IL-6 in peripheral blood of HBV-infected patients are highly expressed and are significantly related to HBV-DNA load and liver function.
