目的 评价SPECT脑血流灌注显像在复杂部分性癫痫灶定位诊断价值。方法 对 35例复杂部分性癫痫患者进行发作间期和发作期SPECT、EEG和MRI检查 ,并进行比较。结果 SPECT、EEG和MRI异常率分别为 88 57% ,68 57%和 42 85 %。 2 3例EEG异常患者与SPECT异常部位基本符合为 1 8例 ,符合率为 78 2 6 % ;1 3例MRI异常与SPECT异常部位基本符合为 9例 ,符合率为 69 2 3%。结论 CPS患者发作间期SPECT绝大多数显低灌注 ,而发作期均表现为高灌注 ;3种检查方法中SPECT对CPS阳性率最高 ,特别是发作期检测临床意义更大 ;由于 3种方法反映癫痫灶不同病理生理变化 ,同时采用可进一步提高定位的准确性。
Objective To evaluate the value of SPECT perfusion imaging in the diagnosis of complex partial epileptic foci. Methods SPECT, EEG and MRI were performed on 35 patients with complicated partial epilepsy during the interictal and exacerbation episodes and compared. Results The abnormal rates of SPECT, EEG and MRI were 88 57%, 68 57% and 42 85% respectively. There were 18 cases with abnormal EEG abnormalities and 18 cases with abnormal EEG. The coincidence rate was 78.26%. The abnormality of 13 cases was consistent with the abnormalities of SPECT in 9 cases and the coincidence rate was 69.23%. Conclusions Most of the SPECT in the interictal CPS patients show low perfusion, while the perfusion is characterized by high perfusion. SPECT has the highest positive rate of CPS in the three examinations, especially in the seizure phase. Because of the three methods Epileptic foci different pathophysiological changes, while using can further improve the accuracy of positioning.
