目的探讨脑出血急性期应用丹参酮ⅡA磺酸钠注射液(简称丹参酮)对脑局部血流动力学的影响。方法将76例基底节区脑出血患者随机分为治疗组和对照组,治疗组在常规治疗基础上加以丹参酮治疗。采用TCD动态观察2组双侧大脑中动脉(MCA)段的血流速度(Vm)、脉动指数(PI)。结果脑出血后2组两侧MCA的Vm均降低,PI均增高,但血肿侧较对侧显著(P<0.05);病后7 d上述改变最为显著(P<0.01),21 d恢复至近病初水平;治疗组脑血液动力学各参数改变较对照组更为显著(P<0.05)。结论丹参酮在脑出血早期应用,确有增加脑血流量,改善低灌注,减少脑损害的作用。
Objective To investigate the effect of tanshinone Ⅱ A sodium sulfonate injection (tanshinone) on cerebral hemodynamics in acute cerebral hemorrhage. Methods 76 patients with cerebral hemorrhage in basal ganglia were randomly divided into treatment group and control group. The treatment group was treated with tanshinone on the basis of routine treatment. The blood flow velocity (Vm) and pulsatility index (PI) of two groups of middle cerebral artery (MCA) were observed by TCD. Results After cerebral hemorrhage, the Vm of MCA in both groups decreased and the PI increased, but the hematoma side was significantly higher than the contralateral side (P <0.05). The changes were most obvious on the 7th day (P <0.01) The initial level of the treatment group, the parameters of cerebral hemodynamics changed more significantly than the control group (P <0.05). Conclusion Tanshinone in the early application of cerebral hemorrhage, does have increased cerebral blood flow, improve hypoperfusion and reduce brain damage.
