目的盆腔包虫病因缺乏特征性而易被误诊,特别是与良性卵巢囊腺瘤鉴别较困难,笔者通过多层螺旋CT(MSCT)平扫及增强扫描提高对盆腔包虫病和不典型良性卵巢囊腺瘤诊断的正确性。方法收集1999年至2009年经手术证实的30例盆腔囊性包块,年龄27~62岁,平均年龄39.5岁。所有病例均行MSCT平扫、增强扫描及多平面重建,并对其CT表现作回顾性分析。结果 5例女性盆腔包虫病,诊断正确率80%,其中2例为单囊型,3例为多囊型(其中1例可见一包虫囊肿破裂)。25例良性卵巢囊腺瘤诊断正确率88%,其中7例为单囊型,18例为多囊型。CT诊断定位正确27例(90%)。结论 MSCT对盆腔包虫病及良性卵巢囊腺瘤的定位、定性诊断及鉴别诊断具有较高的价值。
Purpose Etiology of pelvic echinococcosis is characterized by lack of easy to be misdiagnosed, especially with the identification of benign ovarian cystadenoma more difficult, the author by multi-slice spiral CT (MSCT) plain scan and enhanced scanning to improve pelvic echinococcosis and atypical benign Correct diagnosis of ovarian cystadenoma. Methods Thirty cases of pelvic cystic mass surgically confirmed from 1999 to 2009 were collected. The age ranged from 27 to 62 years with a mean age of 39.5 years. MSCT scan, enhanced scan and multiplanar reconstruction were performed in all cases, and their CT findings were retrospectively analyzed. Results Five cases of pelvic echinococcosis were diagnosed correctly, 80% of which were diagnosed correctly. Among them, 2 were monocystic and 3 were polycystic (1 case showed a hydatid cyst ruptured). The diagnostic accuracy of 25 cases of benign ovarian cystadenoma was 88%, of which 7 cases were single capsular type and 18 cases were polycystic. CT diagnosis of the correct positioning of 27 cases (90%). Conclusion MSCT has a high value for the localization, qualitative diagnosis and differential diagnosis of pelvic echinococcosis and benign ovarian cystadenoma.
