目的:探讨不同部位残胃黏膜幽门螺旋杆菌(Helicobacterpylori,Hp)感染与IL8mRNA表达的关系。方法:对58例残胃患者进行胃镜检查,于吻合口和胃体大弯处,采用两点取材法取材进行胃镜活检。活检标本Hp的感染采用HE和Giemsa染色法检查。活检标本中IL8mRNA表达水平采用实时半定量PCR法检测。结果:58例残胃患者中,38例感染Hp(65.5%)。34例以BI式吻合者中,残胃体部Hp感染阳性者IL8mRNA表达的水平,明显高于Hp感染阴性者的表达水平,分别为0.11±0.07和0.02±0.01(P<0.05);24例BII式吻合者,无论是残胃体部还是吻合口处,Hp感染阳性者IL8mRNA表达的水平,均明显高于Hp感染阴性者的表达水平(吻合口处IL8mRNA表达为:0.32±0.24和0.02±0.01;残胃体部为:0.18±0.08和0.01±0.01)(P<0.05)。结论:残胃黏膜Hp的感染可诱导IL8mRNA表达。残胃体部IL8mRNA的表达主要与Hp感染有关;吻合口处IL8mRNA表达,除与Hp感染有关外,也受胆汁反流影响。
Objective: To investigate the relationship between gastric mucosal Helicobacter pylori (Hp) infection and IL8 mRNA expression in different parts of gastric mucosa. Methods: 58 cases of gastric residual gastroscopy patients, at the anastomosis and gastric curvature, using two materials drawn for endoscopy biopsy. Hp infection biopsy specimens were examined by HE and Giemsa staining. The level of IL8 mRNA in biopsy specimens was detected by real-time semi-quantitative PCR. Results: Of the 58 cases with gastric residual disease, 38 cases were infected with Hp (65.5%). Among the 34 cases with BI-type anastomosis, the level of IL8 mRNA expression in gastric mucosa with Hp infection was significantly higher than that in Hp-negative patients (0.11 ± 0.07 and 0.02 ± 0.01, respectively; P <0.05); 24 In BII anastomosis, the level of IL8 mRNA in Hp-positive patients was significantly higher than that in Hp-negative patients (both anastomosed at the anastomosis of 0.32 ± 0.24 and 0.02 ± 0.01; residual body part: 0.18 ± 0.08 and 0.01 ± 0.01) (P <0.05). Conclusion: Infection of gastric mucosa Hp can induce IL8 mRNA expression. The expression of IL8mRNA in the gastric remnant was mainly related to Hp infection. The expression of IL8mRNA in the anastomosis was also related to the bile reflux in addition to the Hp infection.
