背景:骨盆Ⅱ区肿瘤因其解剖结构复杂,应用计算机辅助技术,个体化手术切除范围设计和植入假体制造可能使临床骨盆骨折治疗效果更为显著。目的:观察计算机辅助技术在骨盆Ⅱ区肿瘤手术中的应用研究及临床效果。方法:对8例肿瘤患者应用CT数据及快速成型技术,模拟骨盆Ⅱ区肿瘤切除术,设计手术切除范围,确定截骨面,模拟修复重建,制造病变骨盆精确模型和个体化假体后,进行手术治疗。结果与结论:手术中病变切除彻底,假体安装顺利,修复精确度高。术后2例患者复发,1例术后2年时出现螺钉松动。参照人工全髋关节置换术后Harris评分标准,肢体功能评分良好。说明应用计算机辅助技术参与骨盆Ⅱ区肿瘤的模拟切除重建,保证了手术的顺利进行和精确的假体重建,临床效果优良,有着良好的应用前景。
Background: Pelvic region Ⅱ tumors may have clinical effects on pelvic fractures because of their complex anatomical structures. The application of computer-aided techniques, the design of individual resection scopes and the manufacture of implanted prostheses may make the treatment of pelvic fractures more obvious. Objective: To observe the application and clinical effect of computer-aided technique in pelvic Ⅱ area tumor surgery. Methods: CT data and rapid prototyping technique were used in 8 cases of tumor patients to simulate resection of pelvis Ⅱ region, to design the scope of surgical resection, to determine the osteotomy surface, to simulate the reconstruction and reconstruction, to manufacture the precise models of pelvic lesions and individualized prosthesis, Surgical treatment. RESULTS AND CONCLUSION: The lesions were completely removed during operation, the prosthesis was installed smoothly and the repair accuracy was high. Two patients relapsed after surgery and one patient had loosening of the screw at 2 years after operation. According to the Harris score after total hip arthroplasty, limb functional score was good. It shows that the application of computer-assisted technology to participate in the resection and reconstruction of the pelvic region Ⅱ tumor to ensure the smooth operation and accurate reconstruction of the prosthesis, the clinical effect is good, has a good prospect.
