目的:探讨实施登记追踪管理与健康教育在患者控制糖尿病中的效果,为更好的开展糖尿病慢病管理提供参考。方法:选取2015年4月~2016年4月浙江省绍兴市立医院门诊或住院确诊为糖尿病的患者共140例,对其进行登记追踪管理与健康教育,观察6个月后患者的糖尿病控制情况。结果:6个月后受试患者的糖尿病控制情况较前明显改善,基本已达到优秀水平。并发高血压病患者的收缩压与舒张压也较受试前明显改善(P<0.01),血脂控制情况较受试前也有显著的下降(P<0.05);受试后患者对于药物的较强依从性明显改善(P<0.01);其主动进行糖尿病的活动,如控制饮食、增加运动的情况,也较受试前有明显改变(P<0.01),并且其战胜糖尿病的信心也较受试前更为坚定。结论:登记追踪管理与健康教育有益于提高糖尿病患者的自我控制能力,并能够有效控制其血糖、血压及血脂等,促进患者康复。
Objective: To explore the effect of implementing registration tracking management and health education in patients with diabetes control, so as to provide a reference for better management of diabetes mellitus. Methods: A total of 140 outpatients or inpatients diagnosed with diabetes from April 2015 to April 2016 in Shaoxing City Hospital of Zhejiang Province were enrolled in this study. Their registration, follow-up management and health education were observed. The control of diabetes was observed after 6 months. Results: After 6 months, the patients under test had significantly improved their diabetes control status, basically reaching the excellent level. Systolic blood pressure and diastolic blood pressure in patients with hypertension were also significantly improved compared with those before the test (P <0.01), and the control of blood lipids was also significantly lower than that before the test (P <0.05) Compliance significantly improved (P <0.01). Active diabetes activities such as diet control and exercise increased significantly compared with those before the test (P <0.01), and their confidence in overcoming diabetes was also significantly improved Before more firm. Conclusion: Enrollment tracking management and health education are beneficial to improve the self-control ability of patients with diabetes, and can effectively control their blood sugar, blood pressure and blood lipids, and promote patient rehabilitation.
