目的探讨血清胱抑素C(Cys-C)、β2微球蛋白(β2-MG)、尿微量白蛋白(Um Alb)、血肌酐(Scr)和尿素氮(Urea)在评估糖尿病肾病中的应用。方法按照轻、中、重度肾功能下降程度将91例糖尿病肾病患者分为A组(46例)、B组(31例)、C组(14例);健康对照组30例,分别测定Cys-C、β2-MG、Um Alb、Scr、Urea,并进行比较。结果 A组5种指标检测结果高于健康对照组,且Cys-C、β2-MG、Um Alb与对照组比较,差异有统计学意义(P<0.05),A组Scr、Urea与健康对照组比较,差异无统计学意义(P>0.05)。阳性率比较:随着肾功能的下降,各指标阳性率增加明显,B组和A组肾功能下降比较,差异有统计学意义(P<0.05)。结论在糖尿病肾病诊断中Cys-C、β2-MG、Um Alb可作为肾损伤早期的指标,其中Cys-C敏感性最高,Scr和Urea在肾损伤早期敏感度不高,只能在一定程度上反映肾脏状态。
Objective To investigate the application of serum Cys-C, β2-MG, Um Alb, Scr and Urea in the assessment of diabetic nephropathy . Methods Ninety-one patients with diabetic nephropathy were divided into group A (n = 46), group B (n = 31), group C (n = 14), and healthy control group (n = 30) C, β2-MG, Um Alb, Scr, Urea, and compared. Results The results of the five indexes in group A were higher than those in healthy control group, and there was significant difference between Cys-C, β2-MG and Um Alb in control group (P <0.05). Scr and Urea in group A were significantly lower than those in healthy control group The difference was not statistically significant (P> 0.05). Compared with the positive rate: with the decline of renal function, the positive rate of each index increased significantly, the renal function decline in group B and group A, the difference was statistically significant (P <0.05). Conclusions Cys-C, β2-MG and Um Alb can be used as early indicators of renal injury in the diagnosis of diabetic nephropathy, of which Cys-C is the most sensitive, while Scr and Urea are not sensitive in the early stage of renal injury, only to a certain extent Reflect the kidney status.
