目的研究分析超声以及母体血清AFP检测在产前胎儿神经管畸形中的应用。方法选取2013年1月~2015年1月在我院实施产前检查的孕妇500例作为研究对象,分别对产妇实施超声检查以及血清AFP检查。观察并比较两种不同检查方式所产生的结果。结果在500例孕妇中,引产或分娩证实有胎儿神经管畸形的共25例,占总数的5.0%。单纯应用彩色多普勒超声(CDFI)诊断出胎儿有神经管畸形症状的共有28例,检出率为5.6%;其中误诊9例,误诊率为1.9%;漏诊6例,漏诊率为24.0%。单纯应用母体血清AFP检测的患者中总共检出有神经管畸形症状胎儿39例,检出率7.8%;其中误诊17例,误诊率为3.6%,漏诊3例,漏诊率为12.0%。而采用两种方式联合应用后总检出胎儿神经管畸形27例,检出率为5.4%;误诊3例,误诊率为0.6%;漏诊1例,漏诊率为4.0%。结论超声及母体血清AFP检查方式在检测胎儿神经管畸形中具有较高的临床应用价值,对于及早发现胎儿神经管畸形,降低出生人口的缺陷率,提高出生人口质量具有十分积极的作用。
Objective To study the application of ultrasound and maternal serum AFP detection in prenatal fetal neural tube defects. Methods From January 2013 to January 2015 in our hospital 500 cases of pregnant women in prenatal care as the research object, respectively, the implementation of ultrasound and maternal serum AFP examination. Observe and compare the results of two different examinations. Results Among 500 pregnant women, 25 cases of fetal neural tube defects were confirmed by induced labor or childbirth, accounting for 5.0% of the total. A total of 28 cases diagnosed as having neural tube defects by color Doppler ultrasonography (CDFI) had a detection rate of 5.6%. Among them, 9 cases were misdiagnosed with a misdiagnosis rate of 1.9%, 6 cases missed diagnosis, and the misdiagnosis rate was 24.0% . A total of 39 fetuses with symptoms of neural tube defects were detected in the patients with simple AFP detection. The detection rate was 7.8%. Among them, 17 were misdiagnosed, with a misdiagnosis rate of 3.6% and 3 missed diagnoses. The rate of misdiagnosis was 12.0%. 27 cases of fetal neural tube defects were detected with the combination of the two methods. The detection rate was 5.4%. Misdiagnosis in 3 cases was misdiagnosis rate was 0.6%. Misdiagnosis was in 1 case and the rate of misdiagnosis was 4.0%. Conclusion Ultrasound and maternal serum AFP test have a high clinical value in the detection of fetal neural tube defects. It has a very positive effect on the early detection of fetal neural tube defects, the reduction of birth defects and the improvement of birth mass.
