目的:通过大样本的临床种植病例,寻求种植体骨愈合的节点,明确什么状态能够得到骨愈合,确定什么状态不能完成骨愈合。方法:实验条件。1.单一德国XIVE种植系统。2.实验医生单一,马泉生,十年以上,稳定团队。3.BRANEMARK手术方法,要求。扭力30牛顿不埋植,4.百级层流净化手术室,无菌,无尘。5.骨量无选择局部适应症,无自体植骨。可拔牙即刻种植,前牙即刻修复。6.2007-2009门诊病例873个种植体。结果:2007-2009门诊病例873个种植体。3-6月修复时回访,失败4个。99.55%。失败原因:失败原因-组织不愈:1.高血糖;2.过劳;3.肾病、肝病等各种原因导致组织不愈。讨论:决定成功的有三个问题:1)种植体形态;2)表面技术;3)植入技术和方法。与Branemark系统相比,锥度带来早期稳定,提高了骨质疏松骨成功率,特殊喷沙酸蚀表面使骨可以与自体骨一样愈合。这样植入时不考虑777的要求,只要把种植体植入活的骨床即可愈合,因此可以减少植骨也可以骨愈合。结论:1.骨愈合第一定律:在人体无自体愈合障碍条件下,种植体能够与有生命的骨组织直接结合,并能支持咬合力。2.XIVE系统的设计基础是Branemark系统,用这个产品要有Branemark系统培训思想。
