目的了解心率变异性(HRV)与体位性心动过速综合征(POTS)的关系。方法应用Holter分析系统检测郑州大学第五附属医院2009年1月-2011年6月确诊的32例POTS患儿(病例组)及体检健康的32例儿童(健康对照组)的心电信号,对各项参数进行分析,了解2组参数之间的差异。结果与健康对照组相比,病例组时域指标中24 h全部正常窦性心搏R-R间期的标准差(SDNN)、24 h全程相邻窦性心搏R-R间期之差的均方根值(rMSSD)和全程记录中相邻R-R间期差值>50 ms的百分比(PNN50)差异均有统计学意义(Pa<0.05),24 h内每5 min正常R-R间期平均值的标准差(SDANN)差异无统计学意义(P>0.05);频域指标中高频功率、低频功率/高频功率二组差异均有统计学意义(Pa<0.05),极低频功率、低频功率2组差异均无统计学意义(Pa>0.05)。结论 POTS患儿HRV活性下降,交感神经和迷走神经处于失衡状态,以迷走神经张力下降为主,存在自主神经功能障碍。
Objective To understand the relationship between heart rate variability (HRV) and postural tachycardia syndrome (POTS). Methods Holter analysis system was used to detect the ECG signals of 32 children with POTS (case group) and 32 healthy children (healthy control group) diagnosed from January 2009 to June 2011 in the Fifth Affiliated Hospital of Zhengzhou University. The parameters were analyzed to understand the differences between the two sets of parameters. Results Compared with the healthy control group, the standard deviation (SDNN) of all normal sinus beats at 24 h and rms SD of the difference of RR between adjacent sinus beats at 24 h (PNN50) between adjacent RR intervals in the whole record were statistically significant (P <0.05). The standard deviation (SDANN) difference of the mean normal RR interval every 5 min within 24 h There was no significant difference between the two groups (P <0.05). There was no significant difference between high frequency power and low frequency power / high frequency power in frequency domain (P <0.05) Significance (Pa> 0.05). Conclusions The activity of HRV in children with POTS is decreased, the sympathetic and vagus nerves are in an unbalanced state, and the predominant vagus nerve tension is decreased. Autonomic dysfunction exists.
