目的探讨维持性透析患者血清成纤维细胞生长因子23(FGF23)和Klotho蛋白与钙磷代谢、动脉硬化和左心室肥厚的关系。方法测定血液透析患者50例(HD组)、腹膜透析患者50例(PD组)和健康体检者30例(对照组)血清FGF23和Klotho蛋白水平,分析其与钙磷代谢、动脉硬化和左心室肥厚的关系。结果 HD组和PD组血清FGF23水平均高于对照组(P<0.05),且HD组的FGF23水平高于PD组(P<0.05)。HD组和PD组Klotho蛋白水平低于对照组(P<0.05),HD组和PD组Klotho蛋白水平差异无统计学意义(P>0.05)。透析患者FGF23水平与磷、钙磷乘积(Ca×P)、全段甲状旁腺激素(iPTH)、颈动脉硬化发生呈正相关(P<0.05或P<0.01),而Klotho蛋白水平与磷、Ca×P、iPTH呈负相关(P<0.05),尚未发现FGF23和Klotho蛋白水平与透析患者左心室肥厚发生有明显相关性(P>0.05)。结论在透析患者中,血清FGF23和Klotho蛋白与钙磷代谢紊乱及动脉硬化的发生相关。
Objective To investigate the relationship between serum fibroblast growth factor 23 (FGF23) and Klotho protein and calcium and phosphorus metabolism, arteriosclerosis and left ventricular hypertrophy in maintenance hemodialysis patients. Methods Serum levels of FGF23 and Klotho were measured in 50 hemodialysis patients (HD group), 50 peritoneal dialysis patients (PD group) and 30 healthy controls (control group). The levels of serum FGF23 and Klotho protein were analyzed. Hypertrophic relationship. Results The levels of serum FGF23 in HD group and PD group were higher than those in control group (P <0.05), and the levels of FGF23 in HD group were higher than those in PD group (P <0.05). Klotho protein level in HD group and PD group was lower than that in control group (P <0.05). There was no significant difference in Klotho protein level between HD group and PD group (P> 0.05). The level of FGF23 in dialysis patients was positively correlated with Ca × P, iPTH and carotid atherosclerosis (P <0.05 or P <0.01), while Klotho protein level was positively correlated with phosphorus, Ca × P and iPTH (P <0.05). No correlation was found between FGF23 and Klotho protein levels and the incidence of left ventricular hypertrophy in dialysis patients (P> 0.05). Conclusion In dialysis patients, serum FGF23 and Klotho proteins are related to disorders of calcium and phosphorus metabolism and arteriosclerosis.
