目的了解在超声胎儿结构筛查发现胎儿心外异常时行胎儿超声心动图检查的必要性。方法回顾性研究2011年1月至12月期间在鼓楼医院产前诊断中心超声室接受胎儿超声心动图检查的367例孕妇,其中355例曾在本院进行过超声胎儿结构筛查,回顾其超声结构筛查发现的阳性结果,评价超声结构筛查发现仅有心外异常的病例有无必要进行胎儿超声心动图检查。结果 367例接受胎儿超声心动图检查的病例中,355例曾在本院先接受了超声胎儿结构筛查,247例因发现胎儿异常而进行胎儿超声心动图检查,其中发现心脏合并心外结构异常96例,仅怀疑心脏结构或血流异常102例,仅发现心外结构异常49例。它们进一步超声心动图检查阳性结果分别为85例、85例和31例,阳性率分别为88.5%、83.3%和63.2%。结论超声胎儿结构筛查发现阳性结果,不论是心脏结构异常或心外结构异常,均有必要进行胎儿超声心动图检查。
Objective To understand the necessity of fetus echocardiography in the detection of fetal extracardiac abnormalities by ultrasound fetal structure screening. Methods A retrospective study of 367 pregnant women who underwent fetal echocardiography at the prenatal diagnostic center of Gulou Hospital from January to December 2011 was retrospectively reviewed. Among them, 355 cases had ultrasound screening of fetal structure in our hospital. Structural screening found positive results, evaluation of ultrasound structural screening found that only cases of extracardiac abnormalities are necessary for fetal echocardiography. Results Of the 367 fetuses examined by echocardiography, 355 had fetal ultrasound screening in our hospital and 247 fetuses had fetal echocardiography due to fetal abnormalities. Among them, cardiac structural abnormalities Of the 96 patients, only 102 cases of cardiac structure or abnormal blood flow were suspected, and only 49 cases of extracardiac structural abnormalities were found. Their further echocardiographic positive results were 85 cases, 85 cases and 31 cases, the positive rates were 88.5%, 83.3% and 63.2% respectively. Conclusion Fetal echocardiography is necessary for the positive results of ultrasound fetal structure screening, both of which are abnormal cardiac structures or abnormal cardiac structures.
