目的探讨不同程度足月新生儿窒息复苏后早期凝血功能的变化及其临床意义。方法检测52例不同窒息程度足月新生儿治疗前后血小板计数(PLT)、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(Fg)及凝血酶时间(TT)水平变化,并与对照组进行比较。结果与对照组比较,窒息组患儿PLT、PT、APTT、Fg以及TT等指标均出现变化(P<0.05),而重度窒息组较轻度窒息组改变更为明显(P<0.05);病情缓解后上述指标均可恢复正常。结论室息新生儿存在出凝血功能紊乱,表现为以高凝状态为主的Pred-DIC。
Objective To investigate the changes and clinical significance of early coagulation function after full-term neonatal asphyxia resuscitation. Methods The PLT, PT, APTT, fibrinogen (Fg) and thrombin time (TT) were measured before and after treatment of 52 full-term newborns with different degree of asphyxia. Level changes, and compared with the control group. Results Compared with the control group, the indexes of PLT, PT, APTT, Fg and TT in asphyxia group were all changed (P <0.05), while those in severe asphyxia group were more obvious than those in mild asphyxia group (P <0.05). After remission of these indicators can be returned to normal. Conclusion There is coagulation disorder in newborn infants with ventricular remodeling, which is Pred-DIC with predominant hypercoagulable state.
