目的:用彩色多普勒血流成像(colordopplerflowimaging,CDFI)技术研究青光眼病人眼血流动力学变化及检测指标的可靠性。方法:采用美国Acuson-128XP型彩色多普勒超声仪测量原发性开角型青光眼(primaryopen-angleglaucoma,POAG)25例,原发性闭角型青光眼(primaryangle-closeglaucoma,PACG)30例和正常人30例的眼动脉(ophthalmicartery,OA)、视网膜中央动脉(centralretinalartery,CRA)、后短睫状动脉(shortposteriorciliaryartery,SPCA)的血流动力学指标。同期测量眼压、血压。结果:①正常组内除年龄大者CRA的搏动指数(pulsatilityindex,PI)较高外,其他各血管血流动力学指标在性别、眼别间无显著差异(P>0.05)。②POAG组血流动力学指标异常。主要表现为CRA、SPCA血管阻力增高(P<0.01),且在视野改变中期明显。③血流动力学指标中阻力指数(resistiveindex,RI)变异最小、最稳定。结论:局部血液循环障碍在POAG发病中起?
OBJECTIVE: To study the ocular hemodynamic changes and the reliability of the detection parameters in patients with glaucoma by color Doppler flow imaging (CDFI). Methods: 25 cases of primary open angle glaucoma (POAG), 30 primary angle-closeglaucoma (PACG) and 30 normal primary angle-angle glaucoma (PACG) were measured by American Acuson-128XP color Doppler. The hemodynamics of ophthalmicartery (OA), central retinalartery (CRA) and short posterior ciliary artery (SPCA) in 30 patients were analyzed. Simultaneous measurement of intraocular pressure, blood pressure. Results: ① In the normal group, except CRA with higher age, pulsatility index (PI) was higher, but there was no significant difference in hemodynamics among the other groups (P> 0.05). ② POAG hemodynamic abnormalities. The main manifestations of CRA, SPCA vascular resistance increased (P <0.01), and mid-term visual field changes significantly. ③ The index of resistance (resistive index) in hemodynamics is the smallest and the most stable. Conclusion: Local blood circulation disorder plays a role in the pathogenesis of POAG?
