目的探讨单核细胞CX3C趋化因子受体1(CX3CR1)表达与冠状动脉狭窄性疾病的关系。方法选择冠状动脉血管狭窄≥50%患者70例、冠状动脉血管狭窄<50%患者23例与无冠状动脉病变的非冠状动脉粥样硬化性心脏病志愿者24例分别为狭窄组、不明显病变组与对照组。用流式细胞术测定单核细胞CX3CR1荧光表达强度,比较3组单核细胞CX3CR1表达水平,进行Spearman相关分析,多因素logistic回归分析,ROC曲线分析相关危险因素。结果狭窄组、不明显病变组、对照组CX3CR1荧光强度分别为4.94%(2.41%,6.58%)、2.02%(1.25%,3.57%)、1.68%(1.09%,2.24%),3组比较有统计学差异(Hc=31.025,P<0.01)。与对照组比较,狭窄组CX3CR1荧光强度明显升高,有统计学差异(Z=4.934,P<0.01)。TG和CX3CR1是冠状动脉狭窄性疾病发病的危险因素(P=0.048,P=0.026)。CX3CR1荧光强度在临界值为2.91%时,敏感性为64.1%,特异性为91.7%,曲线下面积为0.810,CX3CR1对冠状动脉狭窄病变有一定的诊断效力(95%CI:0.728~0.891,P<0.01)。结论单核细胞CX3CR1高表达是冠状动脉狭窄性疾病发病的危险因素,对冠状动脉狭窄性疾病诊断有一定的效力。
Objective To investigate the relationship between the expression of CX3C chemokine receptor 1 (CX3CR1) and coronary artery stenosis in mononuclear cells. Methods Totally 70 patients with coronary artery stenosis ≥50%, 23 patients with less than 50% coronary artery stenosis and 24 patients with non-coronary artery disease without coronary artery disease were divided into two groups: stenosis group, Group and control group. The expression of CX3CR1 was measured by flow cytometry. The expression of CX3CR1 in the three groups of monocytes was analyzed by Spearman correlation analysis, multivariate logistic regression analysis and ROC curve analysis of related risk factors. Results The fluorescence intensity of CX3CR1 in stenosis group, insignificant lesion group and control group were 4.94% (2.41%, 6.58%), 2.02% (1.25%, 3.57%) and 1.68% (1.09%, 2.24% Statistical difference (Hc = 31.025, P <0.01). Compared with the control group, the fluorescence intensity of CX3CR1 was significantly increased in the stenosis group (Z = 4.934, P <0.01). TG and CX3CR1 are risk factors for coronary artery stenosis (P = 0.048, P = 0.026). CX3CR1 had a sensitivity of 64.1%, a specificity of 91.7%, and an area under the curve of 0.810 at a cut-off value of 2.91%. CX3CR1 had a diagnostic value for coronary artery stenosis (95% CI: 0.728-0.891, P <0.01). Conclusion The high CX3CR1 expression in mononuclear cells is a risk factor for the development of coronary artery stenosis and has certain effect on the diagnosis of coronary stenosis.
