目的探讨天冬氨酸蛋白水解酶(Caspase-3)和DIAPH-3蛋白在原发性浸润性乳腺癌(IBC)组织中的表达及临床意义。方法收集该院2005年3月-2010年1月经手术切除且病理证实为原发性IBC患者的癌组织石蜡标本130例,另收集病理镜下诊断正常的癌旁组织标本50例,通过免疫组织化学Elivision法检测所有标本Caspase-3和DIAPH-3蛋白的表达,探讨Caspase-3和DIAPH-3蛋白的阳性表达与患者临床病理特征的关系,并对患者随访5年,观察患者5年生存率。结果Caspase-3和DIAPH-3蛋白在IBC组织中的阳性表达率分别为45.38%(59/130)、41.54%(54/130),明显低于癌旁组织的80.00%(40/50)和86.00%(43/50),差异有统计学意义(P<0.05)。Caspase-3蛋白阳性表达与淋巴结转移数、肿瘤最大直径、临床分期及脉管瘤栓有关,差异有统计学意义(P<0.05);DIAPH-3蛋白阳性表达与淋巴结转移数、临床分期及脉管瘤栓有关,差异有统计学意义(P<0.05)。原发性IBC患者均获得完整随访,5年生存率为73.85%(96/130);Caspase-3阳性表达者的5年生存率为93.22%(55/59),明显高于阴性表达者的57.75%(41/71),差异有统计学意义(P<0.05);DIAPH-3阳性表达者的5年生存率为98.15%(53/54),明显高于阴性表达者的56.58%(43/76),差异有统计学意义(P<0.05)。结论 Caspase-3和DIAPH-3蛋白在原发性IBC患者组织中呈低表达,且与患者的临床病理特征和预后关系密切。
Objective To investigate the expression of Caspase-3 and DIAPH-3 protein in primary invasive breast cancer (IBC) and its clinical significance. Methods The hospital collected from March 2005 to January 2010 surgically resected and pathologically confirmed in patients with primary IBC specimens of 130 cases of paraffin-embedded specimens of cancer and the other pathologically collected normal paraneoplastic tissue samples 50 cases, through the immune tissue The expression of Caspase-3 and DIAPH-3 protein in all samples were detected by Elivision method. The relationship between the positive expression of Caspase-3 and DIAPH-3 protein and the clinicopathological features of the patients was explored. The patients were followed up for 5 years and the 5-year survival rate . Results The positive rates of Caspase-3 and DIAPH-3 in IBC tissues were 45.38% (59/130) and 41.54% (54/130), respectively, which were significantly lower than those in paracancerous tissues (80.00% and 40/50) 86.00% (43/50), the difference was statistically significant (P <0.05). The positive expression of Caspase-3 protein was correlated with lymph node metastasis, maximum tumor diameter, clinical stage and tumor thrombus (P <0.05). The positive expression of DIAPH-3 protein, lymph node metastasis, clinical stage and pulse Tumor suppository, the difference was statistically significant (P <0.05). The 5-year survival rate was 73.85% (96/130) in patients with primary IBC. The 5-year survival rate of patients with positive Caspase-3 expression was 93.22% (55/59), which was significantly higher than that of negative expression 57.75% (41/71), the difference was statistically significant (P <0.05). The 5-year survival rate of DIAPH-3 positive patients was 98.15% (53/54), which was significantly higher than that of negative patients / 76), the difference was statistically significant (P <0.05). Conclusions Caspase-3 and DIAPH-3 proteins are low expressed in the tissues of patients with primary IBC, and are closely related to the clinicopathological features and prognosis of the patients.
